195 <br /> <br /> The Board of Commissioners for the County of Cabarrus met in recessed <br />session in the Commissioners' Meeting Room at the Cabarrus County Governmental <br />Center in Concord, North Carolina on Tuesday, August 16, 1994, at 6:30 P.M. <br /> <br />Present <br /> <br />Chairman: Jeffrey L. Barnhart <br />Vice-Chairman: Carolyn B. Carpenter <br />Commissioners: Sue B. Casper <br /> Arne L. Fennel <br /> Kenneth F. Payne <br /> <br /> Also present were Mr. John V. Witherspoon, County Manager; Mr. Fletcher L. <br />Hartsell, Jr., County Attorney; and Mrs. Frankie F. Bonds, Clerk to the Board. <br /> <br />Chairman Barnhart called the meeting to order at 6:30 P.M. <br /> <br />Request to Renew Transportation Contract with CTS/Mayflower - Mr. Mike Byrd <br /> <br /> Mr. Mike Byrd, Transportation Planner, requested that the transportation <br />contract with CTS/Mayflower be extended until December 31, 1994, with an option <br />to extend the contract from that date for one year. He stated this would allow <br />the contract to be renewed on a calendar year basis, not September 1 of each <br />year. During FY 1993-94 the vans operated 253 days, made 53,387 one-way trips, <br />and drove a total of 281,934 miles. The total cost for County agencies using the <br />services was $261,678.14, including required County funding of $41,552.40, or <br />14.7 percent of the total amount expended. <br /> <br /> In response to questions by Commissioners Carpenter and Casper, Mr. Alan <br />Melvin, Area Manager for CTS/Mayflower, reported that local management problems <br />had been resolved and all vendors had been paid. He further stated that the <br />company would provide transportation for dialysis patients on Saturdays and <br />holidays as long as the clients do not require any type of escalated medical <br />care. There was brief discussion regarding the County's liability and the lease <br />of the vans to CTS/Mayflower. Mr. Byrd reported that there had been two <br />accidents since the company began the transportation service in 1991. However, <br />company employees were not responsible for either accident. <br /> <br /> UPON MOTION of Chairman Barnhart, seconded by Commissioner Fennel and <br />unanimously carried, the Board approved the following extension of the contract <br />with CTS/Mayflower until December 31, 1994. <br /> <br />EXTENSION OF AND AMENDMENT TO <br />HUMAN SERVICES CONTRACT <br /> <br /> This Extension and Amendment is made and entered into as of the <br />1st day of September, 1994, by and between the County of Cabarrus, <br />hereinafter referred to as the "County" and MAYFLOWER CONTRACT <br />SERVICES, INC., formerly known as CTS Management Company, Inc., with <br />its principal office at 5360 College Boulevard, Overland Park, <br />Kansas 66211, hereinafter referred to as "MCS." <br /> WHEREAS, the parties entered into that certain Human Service <br />Contract dated August 28, 1991, which was subsequently extended by <br />written agreement through August 31, 1994, hereinafter collectively <br />referred to as the "Agreement;" and <br /> WHEREAS, the parties wish to further extend the term of the <br />Agreement, <br /> NOW, THEREFORE, it is mutually agreed as follows: <br />1. TERM. The term of the Agreement shall be and is hereby <br />extended for four (4) months, beginning September 1, 1994 and ending <br />December 31, 1994 (the "Extended Term"). This agreement may be <br />further extended for additional periods upon terms which are <br />mutually acceptable to both parties. <br />2. Except as amended herein, all of the terms and conditions set <br />forth in the Agreement shall remain in full force and effect. <br /> IN WITNESS WHEREOF, the parties have executed this Agreement <br />as of the day and year first above written. <br /> <br />MAYFLOWER CONTRACT <br />SERVICES, INC. <br />By: /s/ Kyle S.
