If so, staff will confirm this <br />through the Department of Social Services on a special form for this <br />purpose. <br /> <br />If the potential customer does not qualify for/or is not presently <br />receiving assistance, staff will request that they make application <br />on a separate application which asks for such information as home <br />rental or ownership, monthly household income, number in household, <br />equity in real estate, place of employment and reason for requesting <br />financial assistance. This application will be reviewed by both <br />Liaison Members the Commissioners appointed to the Utility <br />Department, for their approval. <br /> <br />The financing methods will be as follows: <br /> <br /> <br />
