Home and Community Care Block Grant for Older Adults <br />County Funding Plan <br />July 1. 2018 through June 00, 2019 <br />Methodology to Address Service Needs of Low Income (Including Low4ruxur e <br />Minority Elderly), Rural Elderly and Elderly with Limited English Proficiency <br />(Older Americans Acl, Section 005(a)(2)1E)) <br />Community Service Provider: Department of Human Services <br />County: Cabanas <br />The Older Americans Act requires that the service provider attempt to <br />provide services to low-income minority individuals in accordance to their <br />need for aging services. The community service provider shall specify how <br />the service needs of low income, low -Income (including low Income <br />minority elderly), rural elderly and elderly with limited English <br />proficiency will be met through the services identified on the Provider <br />Services Summary (DAAS-732). This narrative shall address outreach and <br />service delivery methodologies that will ensure that this target population is <br />adequately served and conform with specific objectives established by the <br />Area Agency on Aging, for providing services to low income minority, <br />individuals. Additional pages may be used as necessary. <br />Cabanus County Department of Human Services goal Is to incroase partidpation in services and <br />programs by low income elderly and elderly with limited English profidency In the community. DHS <br />provides assistance to all individuals living within the county boundaries requesting services <br />regardless of location within the county. The type and extent of sercies clients receive are based on <br />clients' needs and availability of funding. Outreach by the agency has been accomplished by <br />brochures, community forums, word of mouth and contacts with community paNrem. Outreach is <br />ongoing Requests from c0ents who meet the need for HCCBG services will continue to be <br />accepted. New clients will be added as existing clients leave the program or experience a decrease <br />of units neaded or additional funds are secured. As an agency with multiple funding sources. <br />assessments will be made with each case to be sure that the most appropriate funding Is used to <br />best meet the needs of the clients and that the Older Americana Ad funds ere for the target <br />population groups. <br />Attachment number 11n <br />F-2 Page 46 <br />
