Home and Community Cara Block Grant for Older Adults <br />County Funding Plan <br />July 1, 2018 through June 30, 2019 <br />Methodology to Address Service Needs of Low Income (Including Lowancome <br />Minority Elderly), Rum] Elderly and Elderly with Limited English Proficiency <br />(Older Americans Act, Section 305(aN2NE)) <br />Community Service Provider: Cabanas County Planning&Development <br />County: <br />The Older Americans Act requires that the service provider attempt to <br />provide services to low-income minority individuals in accordance to their <br />need for aging services. The community service provider shall specify how <br />the service netts of low income, low-income (including low Income <br />minority elderly), rural elderly and elderly with limited English <br />proficiency will be mel through the services identified on the Provider <br />Services Summary (DAAS-732). This narrative shall address outreach and <br />service delivery methodologies that will ensure that this target population is <br />adequately served and Conform with specific objectives established by the <br />Area Agency on Aging, for providing services to low income minority <br />Individuals. Additional pages may be used as necessary. <br />The Planning & Development Department's goal is to marginate and provide Housing and Home <br />Improvement Services In conjunction with the Department of Human Services that is funded by <br />the Home antl Community Care Block Grants Coordination of this function will produce a service <br />product that Is both accountable and accessible by the lull scope of clients for which the HCCBG <br />funds were designated. It is our intent to meet the needs of that spedallaad population. The <br />Planning & Development Department will coordinate with the Department of Human Services <br />and other service providers to ensure that the population is served. Alt eligible dients will ba <br />provided access to service. The Planning & Development Department will (allow all program <br />guidelines for service provision and dient treatment, Homes will be rehabilitated for eligible <br />Giants. Improvements include, but are not limited to, handicap accessibility, doorwidenings, <br />minor hoof repairs, atm safety Improvements. Any improvements will be made in compliance <br />with all program regulations. The Planning & Development Department will tike nationals from the <br />Department of Human Services and other service providers of the target population, Outreach is <br />achieved through these programa, word of mouth, and visimg nutrition sites and other special <br />programa designed to address the target populatlon <br />Attachment number 1 N <br />F-2 Page 72 <br />
