Cabarrus County Department of Human Services <br />Consumer Contributions Policy <br />Purpose: The purpose of the Consumer Contributions Policy is to extend the availability of In -Home and <br />Community Based Services administered by the Cabarrus County Department of Human Services by <br />soliciting from recipients of specific services a portion of the cost for services rendered based upon their <br />ability and willingness to contribute. Revenues collected from service recipients will be retained by the <br />Cabarrus County Department of Human Services and used to increase services to existing clients and/or <br />provide in-home and community based services to other individuals in need of such services. <br />Policy & Procedures Reference: The Consumer Contribution Policy and Procedures (effective date: <br />September 1, 2005, last update: February 5, 2013) integrates the provisions of the state and federal laws <br />into a single set of policies and procedures. <br />Services Impacted: The Consumer Contributions Policy applies to the following Home Community Care <br />Block Grant (HCCBG) services ( Adult Day Care, Adult Day Health, In -Home Aide Services, Congregate <br />Meals, r....-•,_._-DiF2Gt-"iH__._ Q, -_ '_.____.__G_ 92-•",.2- HCCBG Transportation), and Family <br />Caregiver Support Program Services (Care Management, Respite and Supplemental Services). <br />Collection Procedures: <br />Adult Day Care/Health Services, Cnnw--• ^'•--'-' F-•••'---, Family Caregiver Support Program <br />(Respite and Supplemental Services) and In -Home Aide Services <br />1. Discuss the Consumer Contributions Policy over the phone when services are requested. <br />2. Issue initial and annual approval notice (Appendix A), service specific provider <br />assurance (Appendix C -G) and the explanation of voluntary contributions handout <br />(Appendix J) with business reply envelopes. <br />3. Issue quarterly Consumer Contributions Policy reminder notices noting total cost of <br />service (Appendix B) with voluntary contributions handout (Appendix J) and business <br />reply envelopes. <br />Congregate Nutrition <br />1. Discuss, review provider assurance (Appendix C) and provide an explanation of <br />voluntary contributions handout (Appendix J) at the meal site explaining voluntary <br />contributions and suggest ways to contribute at initial approval and annual <br />recertification. <br />2. Display a poster in the nutrition site explaining the Consumer Contribution Policy. <br />Page 156 <br />
