a) Receipts reflecting an unusual and/or unexpected expense during the 30 <br /> days preceding the Emergency Assistance application. <br /> b) Social Security Cards for household members. <br /> c) Documentation of citizenship per DHHS Economic and Family Services <br /> Section Energy Manual Section 320. <br /> d) Physician statements for those applicants and/or household members that <br /> have physical or mental impairment that substantially limits their ability to <br /> maintain or secure employment during the 30 days preceding the <br /> Emergency Assistance application. <br /> e) Check stubs, wage statements, ESC benefits, child support, income tax <br /> returns/refunds, bank or retirement fund statements for applicant and all <br /> household members as applicable. <br /> f) Check stubs and/or wage statements reflecting decreased income during the <br /> 30 days preceding the Emergency Assistance application. <br /> g) Utility termination or past due notices. <br /> h) Completed Crisis Landlord Statement <br /> i) Eviction court documents, verification of property condemnation, temporary <br /> housing situation(i.e. homeless shelter, Family Promise, C-VAN)related to <br /> previous residence. <br /> j) Additional documentary proof as deemed necessary. <br /> 10. Services to be provided through EA are as follows, when there are emergency <br /> needs, and are directly related to obtaining or maintaining employment: <br /> • Car repairs (must be employment related) <br /> • Car insurance (must be employment related) <br /> • Car payments (must be employment related) <br /> • Registration Fees for Daycare (must be a licensed daycare and employment <br /> related) <br /> • Uniforms and/or tools when necessary for employment(must be employment <br /> related) <br /> • Background and fingerprint checks (must be employment related) <br /> • Physicals and shots (must be employment related) <br /> • Housing for displaced families in a crisis situation. Only paid when following <br /> conditions exist: eviction from previous residence (eviction court documents <br /> required)property condemnation of prior residence, temporary housing <br /> situation (i.e. homeless shelter, Family Promise, CVAN). No casual moves. <br /> • Past due rent (must have completed Crisis Landlord statement). <br /> • Licenses and/or tests deemed necessary for work under the Work First <br /> program and inability to get license and/or tests will jeopardize employment <br /> • Utility expenses (must be past due) Deposits, returned check charges and/or <br /> tampering fess will not be included.
