ISSUES RELATED TO AGENCY ADOPTIVE PLACEMENT <br />COMPONENTS IN PREPARATION FOR ADOPTION <br />❑ A complete medical history on the birth family (Family Medical History) is to be initiated at time <br />of custody and is vital to foster and adoptive placement services. <br />❑ The child's social worker shall meet with the Placement Specialist to complete the Adoption <br />Assistance checklist no more than 30 days after the child is cleared for adoption. <br />❑ The Adoption Recruitment Strategy is completed for the child and placed in the record unless <br />the child has an identified home at the time of being cleared for Adoption. <br />❑ The child shall be placed on the North Carolina Adoption Resource Exchange within 30 days of <br />the child being cleared for adoption unless the child has an identified permanent home. <br />❑ Sibling placement issues must be addressed via Adoption Committee prior to placement for the <br />purposes of adoption <br />❑ Each child's developmental, physical or mental health assessments or evaluations must be <br />completed and /or updated to provide the most comprehensive information to prospective <br />adoptive families. <br />❑ Mental health counseling services for the child will be initiated (if not already in place) to <br />ensure that adoption - related issues of grief, loss and separation are addressed at a <br />developmentally appropriate level. <br />❑ Lifebook information will continue to be gathered and developed in conjunction with the foster <br />family and any significant persons to the child to ensure that the child's experiences and life <br />events are recorded. <br />❑ Foster parents should be included in a child's preparation for adoption and their role in <br />supporting this transition <br />❑ Preparation of the adoptive family should include providing information about the social and <br />legal implications of adoption, any change they may experience in transitioning from foster to <br />adoptive parent and education about the grief, loss and separation issues that are a normal <br />part of a child's experience when not returning to the birth family. <br />❑ Prospective adoptive parents must be informed as to the differences between foster care <br />placement and adoptive placements and have the role of Adoption Committee in this process <br />outlined. <br />❑ Preparation of the Child Adoption Profile with a detailed social history and all required <br />components. <br />❑ Prospective adoptive applicants will complete fingerprinting required for State and Federal <br />Criminal records checks. <br />❑ Child's eligibility for Adoption Assistance is made without regard to the adoptive family income <br />and will be discussed with prospective adoptive family prior to placement. <br />1303 S.
