3 <br />Adoption Committee: Duties, Membership and Decisions <br />The agency director has the legal responsibility and authority to make adoptive <br />placements for children in the agency's legal custody. The director has delegated <br />this responsibility to the Adoption Committee. Adoption Committee ensures that <br />timely permanence is achieved and addresses any barriers to permanence for <br />children through the following functions: <br />❑ Determining sibling separation <br />❑ Approving or denying preplacement assessments for prospective families <br />❑ Selecting adoptive placement of children with approved adoptive families <br />❑ Reviewing cases in which adoption placement has disrupted and /or a petition for <br />adoption was dismissed to assess the strengths and needs of that particular <br />adoptive placement <br />Cases can be brought to Adoption Committee as long as at least one prospective family has <br />been identified or for a specific purpose as noted above. <br />The 3 rd Wednesday of each month at 8:30 am is held open for Adoption Committee should <br />cases need to be scheduled. Adoption Committee presentations are scheduled through our <br />Adoption Social Worker, who then sends notification and materials out to Committee <br />members. The referring social worker is responsible for coordinating with the Adoption Worker, <br />the presentation of a family's Preplacement Assessment prior to scheduling for Adoption <br />Committee. <br />ADOPTION COMMITTEE MEMBERSHIP <br />The following are standing members of the committee: <br />❑ Children's Services Supervisor (2); <br />❑ Foster Care Program Manager or Child Welfare Program Administrator; <br />❑ Community representative with knowledge of adoptions; <br />❑ Adoption Social Worker; <br />❑ Child's Social Worker; <br />❑ GAL. <br />1303 S.
