CONFIDENTIAL DRAFT CONFIDENTIAL DRAFT <br />EXHIBIT A <br />CHS SERVICES <br />CHS will provide to Employer the following personnel who will perform the services listed <br />below at the Facility from 8 a.m. to 5 p.m., Monday through Friday (or such other schedule as mutually <br />agreed upon by the parties): <br />Type <br />FTE Status <br />Advanced Clinical Practitioner <br />1.0 <br />Registered Nurse <br />1.0 <br />Registrar <br />1.0 <br />In no event shall CHS's employees be required to work on holidays recognized by Employer. <br />Additional services and /or hours may be added to this Agreement by written amendment <br />executed by the parties. <br />CHS Services <br />1. Educate Clinic Users to become better consumers of healthcare. <br />2. Provide diagnosis and treatment of acute episodic illness such as cold and flu symptoms, ear and sinus <br />infections, and seasonal allergies. <br />3. Provide disease management resources to complement primary care physician's direction of care of <br />chronic medical conditions such as asthma, diabetes and hypertension. <br />4. Upon request, assist Clinic Users in locating a primary care physician or other healthcare professionals. <br />5. If a Clinic User does not have a primary care physician, write appropriate prescriptions and administer <br />medications under the supervision of a CPN physician. Absent permission obtained from a Clinic User's <br />primary care physician, prescriptions should typically be for acute diagnosis and not interfere with <br />medications currently prescribed by a Clinic User's primary care physician. <br />6. Document all Clinic User encounters, records, observations, interventions and actions in a medically <br />appropriate manner on Clinic Users' confidential medical records. <br />7. Enroll Clinic Users for subscriptions to CHS publications regarding health - related news. <br />8. Assist Employer with establishing a link on the CHS website exclusive to Clinic Users for individual, <br />password - protected, access to their personalized health- related information. <br />9. Order necessary radiographic and other imaging and prescribe medications and physical modalities as <br />needed. Order necessary special procedures such as cardiac stress tests, ambulatory monitoring of blood <br />pressure or cardiac rate and rhythm and electroencephalograms. Refer Clinic Users, when necessary, for <br />chest x -rays (2 view). <br />10. Incision and drainage of skin abscesses. <br />11. Physical exams (ex.
