Provide on -site laboratory tests and interpret results for blood and urine, such as blood glucose, <br />cholesterol, lipoproteins, antigens for streptococcal organisms; pregnancy; influenza; mononucleosis; <br />urine glucose, protein, or infection ( "dip urinalysis ") on a case -by -case basis. <br />13. Provide education and resources on Lifestyle Behaviors including counseling for cessation of tobacco, <br />alcohol and illicit drug use; medical instructions to patients including diet and exercise. <br />14. Provide immunizations (such as tetanus) and perform below- the -waist physicals. <br />15. Access to a Nurses line (technically a 24 -hour service) for after -hours assistance. <br />16. Access to Virtual Visits with CHS providers for after -hours assistance (additional fees apply). <br />17. Treatment of Worker's Compensation (initial and follow -up as prescribed). <br />18. Drug and alcohol testing as required under DOT guidelines. <br />19. Screenings for bladder cancer. <br />20. Screenings for lead levels (OSHA). <br />21. Biometric screenings and Health Risk Appraisals: Services will include measuring height, weight, <br />blood pressure, waist circumference, lipid profile and Hemoglobin AIc by way of finger stick. <br />22. PCP Form Processing: CHS will process screening results (provided on forms acceptable to CHS) <br />from Clinic Users' primary care physicians. <br />23.
