Describe how the Board acts on monitoring, assessment and evaluation reports. <br />The Board of Directors reviews all monitoring, assessment and evaluation reports. The board <br />also reviews corrective measures and ensures that policies and procedures are modified <br />based on the reports that are received. The Board also ensures that the results of the <br />assessment are put into an action plan to improve the agency's performance. <br />4. Describe the Board's procedure for conducting the agency self - evaluation. <br />The Board of Directors along with staff annually evaluation of program governance, <br />management systems, fiscal, partnership engagement and program effectiveness. The <br />evaluation is documented and a work plan is completed. <br />5. Summarize the results of the Board's most recent self - evaluation. Describe how the <br />information has been or will be used to develop the agency's next Strategy for Eliminating <br />Poverty. Indicate the timeframe and planned activities for the next evaluation. <br />The results from the evaluation include: a) the need to increase and enhance our service delivery <br />to the Hispanic community, b) improve our partnership collaborations around education and <br />connectivity to housing, c) the need to expand funding beyond federal dollars. The information from <br />the last self - evaluation information is included in the strategic plan, departmental work plans and <br />staff performance plans. The information is also used to develop training for the next year, (board <br />and staff).
