employer's liability and automobile insurance have one million dollars in coverage limits and is also covered <br />under our ten million dollars excess umbrella. <br />PROJECT DOCUMENTATION: <br />CCI will prepare and maintain complete and precise documentation of the work. This information, in addition to the <br />administrative documents, will be contained within our closeout package and includes: <br />1. Insurance Certificate <br />2. North Carolina Contractor's License <br />3. Daily Roster Forms <br />4. OSHA personal air sampling and TCLP (one sample) results <br />CLARIFICATIONS: <br />1. Pricing stated within this proposal is based upon: <br />a. EPA and OSHA regulations in effect as of the date of this proposal. <br />b. State of North Carolina requirements in effect as of the date of this proposal. <br />2. Utilities to be supplied by others to CCI, include power, water, and access to sanitary sewer on site (at no cost to <br />CCI). <br />3. Industrial hygiene services are not included in the above - referenced proposal. <br />4. Costs associated with replacement materials are not included in this proposal. <br />5. Access to regulated work areas will be restricted for the purpose of safety and as required by law. Personal protective <br />equipment, including respiratory protection, will be maintained and made available at the site to authorized parties. <br />All individuals entering work areas, once removal and /or repair operations are underway, are subject to requirements <br />of Federal, State, and Local regulations. <br />6. Performance and Payment Bonds are not included in the above - referenced proposal. <br />7.
