o Hours or service per agency <br />o Miles of service per agency <br />• Daily, Weekly, and Monthly Management Report- <br />Sum number of unduplicated passengers served, one -way trips, hours available for <br />service, vehicle hours and miles, accidents per mile, complaints per mile, etc. <br />Contractor shall be expected to perform their regular contracted services 24 hours a day, seven days a <br />week. 48 hours' notice will be given for weekend trips and those trips falling outside normal business <br />hours. <br />The Contractor shall be excused from performance hereunder during the time and to the extent that <br />he/she is prevented from performing by acts of God, strike, and or confiscation of vehicles, materials, <br />products, plants, or facilities. The County reserves the right to assign the areas covered by this <br />Agreement to another contractor either in whole or in part, whenever the Contractor is unable to <br />perform. <br />The Contractor shall transport approved, properly authorized individuals to a specified destination upon <br />request, on time, and in accordance with arrangements made by CCTS. <br />Clients shall be provided door to door transport on routes between designated pick -up locations and <br />assigned facilities. Transportation will be provided for ambulatory, non- ambulatory and wheelchair; <br />Geri -chair residents, based on assessed need. <br />The Contractor shall also provide transport for approved attendants to ride with and assist authorized <br />passengers. One attendant per individual may accompany the authorized passenger free of charge. <br />Attendants shall include but not be limited to parents, spouses, other family members, or care givers. <br />Caregivers may include friends, neighbors, acquaintances, paraprofessionals or professional care <br />providers. The contractor will not supply attendants. Rider certified as requiring an attendant will be <br />transported only when accompanied by an aide. <br />The Contractor's drivers are required to ensure that each passenger properly utilizes his /her seat belt <br />when riding in the Contractor's vehicle and for ensuring compliance with federal safety laws for <br />seatbelts and child safety seats.
