o Trip Request Time <br />o Client Name <br />o Passenger clarifications (elderly, disabled, etc.) <br />o Pickup location <br />o Drop off location <br />o Pickup time <br />o Drop off time <br />o Trip mileage <br />o Pay rate <br />o Amount billed <br />o Wait time charges detailed <br />o Pre -trip and Post -trip inspections sheets for each vehicle performed daily <br />• No -shows reported and DSS19 slips collected for the previous week <br />• The trip sheets are to be kept by the Contractor for five years after the day of service and shall <br />be available to CCTS for inspection upon request. <br />• Any and all accidents and/or incidents while transporting CCTS authorized passengers shall be <br />reported to CCTS immediately and the written report submitted within 24 hours of the accident <br />and /or incident. <br />• The Contractor will be responsible for properly maintaining separate records and summaries for <br />this service as deemed necessary. The name and description of the hardware or software used <br />to obtain the records and create reports should be submitted. <br />The following are types of information, which COTS could require the Contractor to keep. <br />• Daily Trip by Trip Records- <br />• Trip origin and destination address <br />• Clock time, vehicle odometer reading, and on -board passenger count at time of <br />passenger pickup and drop off <br />• Daily, Weekly, and Monthly Reports - <br />a By Agency: <br />• Passenger count by agency <br />13 <br />Attachment number 1 <br />F -4 Page 58 <br />
