occurrence resulting in personal injury, bodily injury, including death, sickness, and disease to any <br />person; or damage or destruction to property, real or personal arising directly or indirectly from <br />operations, products or services rendered or purchased under this contract. <br />It is specifically understood that the Contractor will not transport any person until Contractor has <br />obtained all information necessary for billing purposes including name, address, and phone number for <br />the person to be transported. In the event the Contractor fails to obtain necessary billing information, <br />the county shall not be responsible for paying The Contractor's invoice with respect to such transport. <br />Maintenance Requirements <br />• The contractor shall ensure that a Vehicle Maintenance Manager is responsible for the <br />development and execution of a comprehensive base wide vehicle maintenance program <br />• The Maintenance Manager shall be experienced ten years or greater in the automotive work <br />field and able to determine /resolve technical matterslproblems involved in the repair and <br />maintenance of vehicles and equipment. <br />• The overall goal of the program should be to identify and correct any problems that may exist <br />before they are identified by or reported to CCTS and/or the vehicle maintenance staff. <br />• The contractor shall provide repair and replacement of vehicle and equipment tires in <br />accordance with CCTS SSSP (State Systems Safety Plan) directives and manuals. <br />• The contractor shall maintain all vehicles to the standards required by the CCTS SSSP <br />mandatory directives and manuals. <br />• Unless otherwise specified herein, the contractor shall provide all supplies, personnel, <br />equipment, tools, materials, supervision, and other items or services necessary to perform the <br />management and operation of motor vehicle maintenance functions as defined. <br />Reporting Requirements <br />• On the first working day of each week, CCTS shall receive an activity report, no later than 11:00 <br />am, including the following information for the previous week. <br />• For each day of the week, a record of each trip provided including: <br />o Date <br />o Trip# <br />12 <br />Attachment number 1 <br />F -4 Page 57 <br />
