Agency Name <br />3. Describe how the Board acts on monitoring, assessment and evaluation reports. <br />The Board of Directors reviews all monitoring, assessment and evaluation reports. The <br />board also reviews corrective measures and ensures that policies and procedures are <br />modified based on the reports that are received. The Board also ensures that the <br />results of the assessment are put into an action plan to improve the agency's <br />performance. <br />4. Describe the Board's procedure for conducting the agency self - evaluation. <br />The Board of Directors along with staff annually evaluation of program governance, <br />management systems, fiscal, partnership engagement and program effectiveness. The <br />evaluation is documented and a work plan is completed. <br />5. Summarize the results of the Board's most recent self - evaluation. Describe how the <br />information has been or will be used to develop the agency's next Strategy for Eliminating <br />Poverty. Indicate when the next evaluation will begin and be completed. <br />The results from the evaluation include: a) the need to increase and enhance our <br />service delivery to the Hispanic community, b) improve our partnership collaborations <br />around education and connectivity to housing, c) the need to expand funding beyond <br />federal dollars. The information from the last self - evaluation information is included in <br />the strategic plan, departmental work plans and staff performance plans. The <br />information is also used to develop training for the next year, (board and staff).
