To resist any effort to obtain access to client treatment information not <br />expressly provided for in the aforementioned paragraph. <br />3. Provider shall obtain appropriately signed forms allowing for the <br />release of information that may be necessary for the Provider to <br />conform to the requirements of the NC Criminal Justice Partnership <br />Program and all Federal Confidentiality regulations. <br />SECTION XI1: EQUIPMENT COSTS AND OFFICE SPACE <br />The Provider shall supply, at its own expense, all equipment, tools, materials <br />and /or supplies required to deliver treatment services contracted hereunder. The <br />Provider shall provide office space for necessary staff members. <br />SECTION XII1: CLIENT ADMISSION <br />In accordance with the provisions of Title IV of the Civil Rights Act of 1963, and <br />the regulations of the United States Department of Health and Human Services, <br />issued thereto, the Provider agrees to extend all funds allocated in such a <br />manner that no person will be excluded from participation in, be denied the <br />benefits of, or otherwise be subject to discrimination under purchased services <br />on the grounds of race, religion, color, national origin or sex. <br />Attachment number 1 <br />F -4 Page 72 <br />
