to explain any negative financial information in its financial statements and are encouraged to <br />provide documentation supporting those explanations. <br />All financial information, statements and /or documents provided in response to this Invitation <br />to Submit Applications requirement shall be kept confidential, IF THE APPLICANT <br />COMPLIES WITH PARAGRAPH 13 OF THE GENERAL INFORMATION ON SUBMITTING <br />APPLICATIONS BY MARKING THE FINANCIAL INFORMATION, STATEMENTS AND /OR <br />DOCUMENTS CONFIDENTIAL. <br />5. Insurance Certificates <br />The applicant must provide certificates demonstrating insurance coverage for each of the <br />following policies with coverage NOT LESS than the amounts specified below: <br />— Workers' Compensation and Employer's Liability Coverage must cover all the <br />subcontractor's employees who are engaged in work under the contract; minimum <br />limit of $500,000. <br />— Commercial General Liability Coverage on a Comprehensive Broad Form on a on an <br />occurrence basis in the minimum amount of $1,000,000 Combined Single Limit. <br />Defense cost shall be in excess of the limit of liability. <br />— Automobile Liability shall cover all owned, hired and non -owned vehicles used in the <br />performance of this contract. The minimum combined single limit shall be $500,000 <br />for bodily injury and property damage, $500,000 for uninsured /under - insured <br />motorist, and $25,000 for medical payment. <br />— Pollution Occurrence Liability Insurance to protect against incidental disturbances of <br />environmental pollutants like lead -based paint dust. The policy must provide three <br />basic limits: $500,000 per occurrence, $500,000 aggregate for the policy term and <br />$2,500 deductible per occurrence. <br />— Workmen's Compensation Insurance must include an employer's liability coverage <br />endorsement with a minimum limit of $100,000. The insurer must be bound to notify <br />both the company and the subcontractor at least ten (10) days prior to termination of <br />such insurance, or any portion thereof. <br />6. Corporate Conflict of Interest Policy <br />The applicant must provide a copy of the company's policy on Conflicts of Interest. At a <br />minimum, the policy must require that: <br />— No subcontract or procurement will be made with any organization in which any <br />person in an administrative capacity may benefit unless specifically authorized in <br />writing by the contracting agency. <br />— No subcontract or procurement will be made with any organization in which the <br />immediate family of a person in an administrative capacity with the Subcontractor <br />may benefit unless authorized in writing to do so by the contracting agency. <br />— No employee or official of the subcontractor shall solicit or accept gratuities, favors or <br />anything of monetary value from contractors, providers or potential subcontractors. <br />7. Other Attachments <br />In this section, the applicant may provide additional information to support their Application, <br />such as resumes or training certificates for its employees. This section is optional. Note that <br />no new information should be included in this section since it is not required to be reviewed <br />as part of the evaluation. <br />APPLICATION FORMAT SPECIFICATIONS <br />1. All paper copies should be printed double -sided and on recycled paper with a minimum post - <br />consumer content of 30 percent. <br />2.
