8. Each applicant for EA shall furnish and /or assist the worker to verify his eligibility by presenting documentary proof where required verifying needs, income, the availability of other resources, and any other data required for eligibility determination as follows: a) Receipts reflecting an unusual and /or unexpected expense during the 30 days preceding the Emergency Assistance application. b) Social Security Cards for household members. c) Documentation of citizenship per DHHS Economic and Family Services Section Energy Manual Section 320. d) Physician statements for those applicants and /or household members that have physical or mental impairment that substantially limits their ability to maintain or secure employment during the 30 days preceding the Emergency Assistance application. e) Check stubs, wage statements, ESC benefits, child support, bank or retirement fund statements for applicant and all household members as applicable. f) Check stubs and /or wage statements reflecting decreased income during the 30 days preceding the Emergency Assistance application. g) Utility termination notices. h) Leases and eviction notices (court documents or landlord statement indicating that eviction will occur if payment is not received on a specific date). i) Employer statement reflecting termination date and or reason for termination if applicant or household member meets unemployed as defined in Section l.d or f above. j) Eviction court documents, verification of property condemnation, temporary housing situation (i.e. homeless shelter, Family Promise, CNAN) related to previous residence. k) Additional documentary proof as deemed necessary. 9. Services to be provided through EA are as follows, when there are emergency needs, and are directly related to obtaining or maintaining employment: • Car repairs (must be employment related) • Emergency housing for victims of family violence (exceeding DV funding limits) • Housing for displaced families in a crisis situation Only paid when following conditions exist: eviction from previous residence (eviction court documents required) property condemnation of prior residence, temporary housing situation (i.e. homeless shelter, Family Promise, CVAN) This would help to exclude casual moves. • Past due rent (must have an eviction notice or landlord statement indicating that eviction will occur if payment is not received on a specific date) • Licenses deemed necessary for work and inability to get license will jeopardize employment Attachment number 1 F -3 Page 100
