CONTRACT FOR SERVICES PROVIDED FOR <br />UNDER NORTH CAROLINA STATE APPROPRIATIONS <br />(SENIOR CENTER GENERAL PURPOSE) <br />THIS AGREEMENT, entered into as of this 1st day of July , 2007 , by and between Cabarrus County <br />Deparment of Aging, Cabarrus County Senior Center , (hereinafter referred to as the "Agency") and <br />the Centralina Council of Governments (the Area Agency on Aging), (hereinafter referred to as the <br />"Council"); <br />WITNESSETH THAT: <br />WHEREAS, the Council wishes to have performed and the Agency wishes to provide certain aging <br />services'in connection with activities financed in part by state appropriations provided to the Council <br />through the North Carolina Division of Aging and.Aduit Services as set forth in this agreement; <br />NOW THEREFORE, in consideration of these premises, and mutual covenants and agreements <br />hereinafter contained, the parties hereto agree as follows: <br />Employment of the Agency. The Council hereby agrees to engage the Agency and. the Agency <br />hereby agrees to perform the services hereinafter set forth in accordance with the terms and <br />conditions contained herein. <br />2. Scope of Services. The Agency shall provide- Senior Center General Purpose services to <br />individuals who are 60 years of age. or older as specified in the Agency's Budget Summary <br />attached hereto as Exhibit A. All services are to be performed in accordance with the. Agency's <br />County Funding Plan for Aging Services submitted to the Council, .the terms of which are <br />incorporated herein by reference. <br />3. Time of Performance. The services of the Agency are to commence no earlier than July 1, 2007 <br />and shall be undertaken and pursued in such sequence as to assure. their expeditious completion <br />on or before the end of the contract period, June 30, 2008. <br />4. Contract Administrator. The contract administrator for the Council shall be Gayla S. Woody. <br />The contract administrator for the Agency shall be -Mike Murphy It is <br />understood and agreed that the administrator for the Council and the Agency shall represent their <br />respective employers in the performance of this .contract. Each shall notify the other if the <br />administrator changes during the contract period. <br />5. Compensation and Method of Payment. The Agency shall be compensated for-the work and <br />.services. actually performed under this contract by payments to be made monthly by the Area <br />Agency on Aging. However, the total ;compensation and reimbursement to be paid the.Agency <br />under this contract shall not exceed the sum of $ 16404 _ in state grant funds. The obligations of <br />the Council set forth in this agreement for payment of compensation are contingent upon receipt <br />of grant funds by the Council from the North Carolina Division of Aging and Adult Services: <br />F-1 <br />
