Secondly, through the inclusion of organized physical activity as a <br />requirement of day camp programs. <br />Physical Projects: Physical projects will include improved signage on existing greenways, walking <br />tracks, and other areas used by community walkers. Mileage indicators will be installed on six walking <br />trails or geenways. Walkers have requested markers to help them improve their walking program. The <br />markers will also improve safety giving the abiltiy to provide emergency services a more exact location <br />on a trail. <br />6. Map: Provide a single map that visually depicts the target location(s)Isetting(s), and highlight <br />any physical project strategies that can be visually represented. <br />r. Partnership's organizational capacity: Describe the partnership's ability to implement the <br />proposed project. Include the partnership's mission, any physical activity and/or healthy <br />eating initiatives that have been undertaken to date, staff leadership experience, <br />organizational experience, community support, and media collaboration. Also include <br />current community strengths and assets that will help to ensure success of the project. If <br />your partnership was formed in response to this grant opportunity, please explain how you <br />plan to build capacity in a short period of time. (Note: Rafher than doing so here, use the <br />Partners Information Form in Part 111 of this application to describe the roles, strengths, and <br />commitments represented by the organizations and individuals comprising your partnership. <br />This form does not count as part of the six-page proposal narrative.) <br />Although our partnership is forming in response to this grant opportunity most of the <br />partnering agencies are involved with each other in a variety of collaborations. All <br />were involved in the development of the Livable Community Blueprint. Cabarrus <br />County has partnered with both Concord and Kannapolis in park development. <br />Cabarrus County, Concord and Kannapolis Parks and Recreation Departments <br />partner with the Senior Center for Senior Games. These established relationships will <br />be the foundation we build on. <br />Our mission is to increase active living opportunities for the citizens of Cabarrus <br />County through access and education and to provide educational opportunities for <br />lifetime activities for the youth. <br />8. Assessment methods: Describe any community assessments that have been conducted <br />that specifically informed your project proposal. Please also describe any planned <br />community assessments that will inform e.g., the target population, the target setting, and/or <br />one or more 5P strategies. <br />Community assessments are conducted every fow years for Cabarrus County. The most recent one <br />completed in December 2004 reveals that overweight and lack of physical activity are significant <br />issues for both youth and adults in ow community. Through an annual county health report we track <br />health risk behaviors. The 2006-07 report indicates that between 2002 and 2005 the percentages of <br />overweight children and adults have continued to increase and aze well above the target set in the <br />North Carolina 2010 Health Objectives. Further informing ow project proposal is the work done <br />locally to develop a Livable Community Blueprint. The LCB is a 10 year guide to developing the <br />necessary parks, greenways, pedestrains routes, bikeways, and other facilities that will provide ow <br />citizens with access to physical activity opportunities. Both of these assessments and planning efforts <br />are ongoing and will continue to inform us with regard to the target population and settings for this <br />project. <br />9.
