COMMITTEE ACTIVITIES & ACCOMPLISHMENTS <br />• Advocated for the rights of all long-term care residents in Cabarrus County. <br />• Attended the required quarterly training/business meetings with the Centralina Ombudsman <br />Program. <br />• Received additional training at the Centralina Annual Aging Services Conference and <br />Leadership Training Session. <br />•S Educated the community about long-term care issues and promoted community involvement <br />through speaking engagements to groups such as 4H Club and church groups. <br />:• Planned and worked at the Cabarrus County Residents' Rights Celebration Luncheon held at <br />Providence Baptist Church in Harrisburg on October 26`h. Over 150 residents, staff, <br />volunteers, county officials, and dignitaries attended this event. <br />• State guidelines require at least 14 members on the ACH Committee, and may have up to five <br />additional members at the discretion of the County Commissioners. Cabarrus County <br />ACHCAC currently has three (3) members. <br />• The Nursing Home CAC completed 100% of all required Quarterly Visits during the <br />reporting year. The Adult Care Home CAC conducted only 50% of all required Quarterly <br />Visits during the reporting year because of too few committee members (although the <br />committee made 100% of required visits to assigned homes). Homes that were not visited by <br />the Committee were visited by the staff of the Ombudsman Program. <br />• Worked with facility staff, families, and residents toward resolution of grievances and <br />concerns. <br />:• Initiated written and electronic reporting methods as required by the program standards. <br />• Made friendly visits to all facilities. One volunteer visits the facilities at least 6-8 hours every <br />other week. <br />•b Maintained positive relationships with long-term care providers. <br />• Provided information about facilities to individuals in need of facility placement. <br />:• Volunteered at facilities for friendly visits, activities, parties, and other events. <br />•3 Attended presentation of the Cabarrus County CAC 2005 Annual Report to the Cabarrus <br />County Commissioners in March 2006. <br />• Sought to increase membership. <br />Pa e 2 of 4 <br />~-~~. <br />
