(2) Within 45 3A days of receipt of the application, the Executive D'vector shall acknowledge and accept <br />the complete,application; or if not complete, shall retum the application to the applicant with a <br />statement of what additional information is required. <br />(3) The Executiv@ Director may deny anv incomplete application for a wastewater discharge permit if the <br />apqlicant fails to submit all required information within the time specified by WSACC. <br />4.3 Wastewater Discharee Permit Issuance and Conditions <br />~(ej Tentative Determination and Draft Permit <br />(1) WSACC staff shall conduct a review of the application and an on-site inspection of the SIU, <br />including any pretreatment facilities, and shall prepare a written evaluation and tentative <br />determination to issue or deny the wastewater discharge permit. <br />(2) If WSACC staffs tentative deYerminaYion is to issue the permit, the following additional <br />determinations shall be made in writing: <br />(i) Proposed discharge limitations for those pollutants qreflese~ to be limited; <br />(ii) Monitoring reauirements for pollutants which are suspected to be present but which are not <br />proposed to be ]imited at the time the permit is issued. After issuance, the ~ermit ma~be <br />modified pursuant to Section 4.4 (b) to imnose limits on one or more of such pollutants. <br />(iii) Where applicable, a proposed schedule of compliance, including interim dates and <br />requirements, for meeting the proposed limitations to include, but not be limited to, <br />installation of a new monitorin~point and/or flow measuringequipment; and <br />(iv) A brief description of any other proposed special conditions which will have significant <br />impact upon the discharge described in the application. <br />(3) WSACC staff shall organize the determinations made pursuant to paragraphs Q) and (2) above and <br />WSACCs general permit conditions into a wastewater discharge permit. <br />f~ (-tj Permit Svnonsis <br />A fact sheet providing a brief synopsis of the application shall be prepared by WSACC staff for submission to <br />the applicant and, the approval authority and shall be made available to the public upon request. The contents <br />of such fact sheets shall include at least the following information: <br />Q) A sketch and detailed description of the industrial facilities and pretreatrnent facilities including the <br />location of all points of dischazge to the POTW and all established compliance monitoring points. <br />(2) A quantitative description of the discharge described in the application which includes at least the <br />following: <br />(i) The rate or frequency of the proposed discharge; if the discharge is continuous, the average <br />daily flow; <br />(ii) The actual average daily dischazge in pounds per day or concentration of any limited <br />28 <br />~~~ <br />
