(2) Within 45 3A days of receipt of the application, the Executive D'vector shall acknowledge and accept the complete,application; or if not complete, shall retum the application to the applicant with a statement of what additional information is required. (3) The Executiv@ Director may deny anv incomplete application for a wastewater discharge permit if the apqlicant fails to submit all required information within the time specified by WSACC. 4.3 Wastewater Discharee Permit Issuance and Conditions ~(ej Tentative Determination and Draft Permit (1) WSACC staff shall conduct a review of the application and an on-site inspection of the SIU, including any pretreatment facilities, and shall prepare a written evaluation and tentative determination to issue or deny the wastewater discharge permit. (2) If WSACC staffs tentative deYerminaYion is to issue the permit, the following additional determinations shall be made in writing: (i) Proposed discharge limitations for those pollutants qreflese~ to be limited; (ii) Monitoring reauirements for pollutants which are suspected to be present but which are not proposed to be ]imited at the time the permit is issued. After issuance, the ~ermit ma~be modified pursuant to Section 4.4 (b) to imnose limits on one or more of such pollutants. (iii) Where applicable, a proposed schedule of compliance, including interim dates and requirements, for meeting the proposed limitations to include, but not be limited to, installation of a new monitorin~point and/or flow measuringequipment; and (iv) A brief description of any other proposed special conditions which will have significant impact upon the discharge described in the application. (3) WSACC staff shall organize the determinations made pursuant to paragraphs Q) and (2) above and WSACCs general permit conditions into a wastewater discharge permit. f~ (-tj Permit Svnonsis A fact sheet providing a brief synopsis of the application shall be prepared by WSACC staff for submission to the applicant and, the approval authority and shall be made available to the public upon request. The contents of such fact sheets shall include at least the following information: Q) A sketch and detailed description of the industrial facilities and pretreatrnent facilities including the location of all points of dischazge to the POTW and all established compliance monitoring points. (2) A quantitative description of the discharge described in the application which includes at least the following: (i) The rate or frequency of the proposed discharge; if the discharge is continuous, the average daily flow; (ii) The actual average daily dischazge in pounds per day or concentration of any limited 28 ~~~
