If services are <br />delivered at multi e 10 'ations, please include information for all locations. <br />Home Based Services are provided primarily in the family's home. This feature of the program is helpful in <br />eliminating many barriers to services. Bringing the counseling to the family's home makes it possible for all <br />family members to participate, and creates a more open environment, as opposed to a more sterile, clinical <br />setting. The Home Ba$ed Counselor has flexible work hours to meet the needs of clients. Furthennore, seeing <br />the family in their "natural setting" allows them to be more comfortable, and they often display more of their true <br />selves. Home Based Qounselors are able to meet with the family in other settings, such as schools or other <br />agencies in the comm4/1ity, when appropriate. The actual offices for the Home Based staff will be located <br />within the Department ~f Social Services, 1303 S. Cannon Blvd. Kannapolis, NC 28083. <br /> <br /> <br />(2) Identify paid or volllnteer staff qualifications, (certifications, degrees, work experience) and their <br />responsibilities relative ;to this component. <br />Home Based Services pounselorslsoclal workers are Master's Degree Level Social Workers andlor meet the <br />state's criteria for a So\;ial Worker Level III, relating to their assessment and counseling intervention skills. All <br />staff have several yearll experience working specifically with the adolescent population and have training In <br />conducting visits in the!home setting. All staff have completed Family Preservation training, focusing on <br />building on family strengths and utilizing the family centered approach in counseling and working with families. <br />Staff have also receivel:l training in Parenting tools and techniques to reinforce with families. Our staff is <br />knowledgeable of resources and agencies within the community to assist in meeting the needs of families. <br /> <br />,n:;;II''': COMPONENT NARRATIVE CONTINUED attach for each com anent <br />3 Describe the see referral, screenin , admission and termination ures. <br />Referrals are recelv . from DJJ by the Home Based Supervisor who assigns referrals to Home Based staff for <br />assessments. Once the referral is assigned, the Home Based Counselor attempts to contact the parent by <br />phone and/or letter. If t. 0 response is received after a five day pertod, another letter is sent requesting contact. <br />If contact is not made ' final letter is sent telling the parent that their referral for services will not be pursued <br />further if contact is notlmade by a final deadline. The DJJ counselor is also made aware of this final attempt. If <br />contact is made with t~e family, a home visit is scheduled. The family is screened for appropriateness, based <br />on their needs and wlllJngness to resolve issues in the home. In other words they must be receptive to working <br />on Identified issues an~ preventing out of home placement If possible. Program guidelines are explained in <br />detail. The assessmerit Involves gathering infonnation of what is currently taking place in the home as well as <br />family history, inCluding what has been tried In the past in addressing issues. Goals and objectives are <br />gathered. If the family fs in agreement to service Intervention and meeting consistently with the Home Based <br />Counselor, the case i~"then opened for services. The duretion ofthe service will last up to six months, <br />contingent on the fami y's progress and willingness to meet and work on Issues. Goals are assessed and <br />reassessed with the f,' i1y as time progresses. Since this is a voluntary service, if the family decides during the <br />course of intervention hat services are no longer needed or perhaps goals and objectives are met, the case <br />will close. Prior to cas' closure, the counselor works with the family on moving towards self-sufficiency.
