<br />ADULT DAY SERVICES AGREEMENT <br /> <br />Cabarrus County Department of Aging <br />Coltrane L. I. F. E. Center, Inc. <br /> <br />This agreement is entered into by the Coltrane L. I. F. E Center, Inc., hereinafter referred to as <br />"the Contractor" and the Cabarrus County Department of Aging, hereinafter referred to as "the <br />County". <br /> <br />The Contractor and the County, in consideration of the mutual covenants, promises and <br />agreements herein contained, agree as follows: <br /> <br />The Contractor shall provide to the County the services set forth in the Agreement documents <br />consisting of providing Adult Day Services for the County for the period of July 1, 2006 through <br />June 30, 2007. <br /> <br />1. RESPONSIBILITY OF THE COUNTY: <br /> <br />The County shall: <br /> <br />I. be the "Community Service Provider" as defined by the North Carolina Division of Aging & <br />Adult Services; <br /> <br />2. assume full responsibility for determining client eligibility and priority status for placement. <br />The County will conduct screening and intake assessments and process eligible clients based <br />on availability of funding resources. However, it is agreed that the Contractor will have the <br />final authority to accept or deny services to any applicant, based on physical, social and/or <br />emotional characteristics that may exist. The Contractor must have County approval prior to <br />initiating services; <br /> <br />3. review "consumer contribution" policies with the client in an effort to encourage voluntary <br />contributions from the client/care giving family. The County will collect and process all <br />contributions from the client/caregiving family in accordance with the NCDAAS policy; <br /> <br />4. conduct assessment site visits on the Contractor as necessary to insure compliance with the <br />appropriate standards and/or policies as required by the various funding resources; <br /> <br />5. conduct client satisfaction surveys in an effort to enhance services through this program; <br /> <br />6. provide technical assistance to the Contractor as needed. <br /> <br />F-5 <br />
