<br />Adult Day Services Agreement <br />Coltrane L. I. F. E. Center <br />Page 2 of 4 <br /> <br />II: RESPONSIBILITY OF CONTRACTOR: <br /> <br />The Contractor shall: <br /> <br />J. be a "direct service provider" for any County approved Cabarrus County resident(s) that <br />choose to receive Adult Day Services from the Contractor; <br /> <br />2. make available to the County any/all records relating to clients that receive Adult Day <br />Services from the Contractor through this agreement; <br /> <br />3. provide the County with a copy of a Certificate of Insurance that will verify adequate <br />current insurance coverage. Furthermore, the Contractor agrees to maintain in effect full <br />insurance coverage, in amounts no less than the industry standard; <br /> <br />4. maintain appropriate Certification by the State of North Carolina to operate an Adult Day <br />Services program; <br /> <br />5. notify the County immediately (same day) in the event of any adverse action regarding <br />Certification that may place the Adult Day Services program in a provisional, revocation <br />or terminated status of operation. The Contractor will continue to keep the County <br />updated and informed concerning corrective action utilized to address deficiencies with <br />complete corrections made within the specified time frame to the State of North Carolina <br />standards; <br /> <br />6. secure at no expense to the County, all required personnel and appropriate professional <br />training to meet the certification requirements necessary to operate this Adult Day <br />Services program; <br /> <br />7. operate this Adult Day Services program a minImum of five days per week and a <br />minimum of six hours per day, Monday through Friday, except for designated holidays, <br />staff training days and inclement weather conditions, in which most cases the Cabarrus <br />County Schools policy will be followed. (Any additional closings must be pre-approved <br />by the County.) The County agrees to reimburse the Contractor for eligible units of <br />service provid+d on Saturdays, to approved clients under this agreement and in <br />compliance with all other terms and conditions included in this agreement. <br /> <br />8. assist the County with the final assessment and the development of the personal care plan <br />of each client; <br /> <br />9. following the screening/intake process and an orientation visit, if enrollment into this <br />program is apProved, a schedule will be established and the contractor will be eligible for <br />reimbursement beginning with the first full day of attendance; <br /> <br />10. in the event th~ Contractor receives consumer contributions from any client/caregiving <br />family, all revertues will be forwarded to the county immediately. <br /> <br />1 J. provide these Adult Day Services in accordance with the North Carolina Division of <br />Aging & Adult Services - Adult Day Service Standards. <br /> <br />f'-5 <br />
