<br />Contract # <br />Easter Seals UCP North Carolina, Inc. <br /> <br />Duties of the Contractor: <br />I) Provide the County with documentation of all costs associated with the payment <br />of Participants' emplqvees including. but not limited to wages. withholdings. taxes. and <br />other emplover relate~ expenses. <br />2) Deliver an invoic~ to the Countv detailing all amounts due to the Contractor no later <br />than seven business davs following the end of each pav period. <br />3) File and remit paVlnent for all taxes associated with payroll expenses on behalf ofthe <br />Participants. <br />4) Based on the hour~ reported bv the County to the Contractor before the end of the <br />third business day fol\owing the end of the prior pay period. the Contractor will prepare <br />and mail paychecks fqr all Participants' employees to addressCs) specified bv the Countv. <br />All pavchecks will beimailed two business days prior to the pav date provided proper <br />approval has been delivered bv managing entitv. <br />5) Prepare. file and d~liver W-2's to all Participants emplovees that received wages <br />during the calendar war. <br />6) Assure that record~ are available for periodic audits. <br />7) Designate a person, emploved bv the Contractor. who shall serve as a primary contact <br />for the Countv. <br />Other Terms of Con~ract: <br />I) Contract can be teqninated with a 30 day written notice by either partv. <br />2) Contract shall be i~ effect from Julv 1. 2006 through June 30. 2007. <br /> <br />C. Rate per unit of Service (define the unit): <br /> <br />I. If Standard Fixed Rate, Maximum Allowable, (See Rates for Services Chart) <br /> <br />$12.50/Unit fot Financial Intermediary Fee. $14.40/hour ($3.60/unitl for Personal <br />Assistant. plus !applicable taxes. <br /> <br />2. Negotiated County Rate. <br /> <br />$35/Participantper month. Average $9.19/hour C2.30/unitl for Personal Assistant. <br />plus applicableitaxes <br /> <br />D. Number of units to ~e provided: 1.139.60 Financial Intermediarv units and <br />199.033.34 Personal Aj'lsistant units <br /> <br />E. Details of Billing process and Time Frames: Deliver an invoice to the Countv <br />detailing all amounts dUe to the Contractor no later than seven business days following <br />the end of each pay pe*od. <br /> <br />F.
