Life Skill Workshops - Whereas the provider agrees to provide each offender <br />assessed appropriate for lOP, Rap, or CBI an eight topic course iflife skills. <br />The topics shall include, but not be limited to, Assertiveness Training, <br />Finances, Parenting, Job Preparation, and Physical Health, This course can be <br />taken at any time during the offenders' treatment. Completion of the life <br />skills workshops will be required in order for the offender to complete <br />treatment. <br /> <br />R. GED- The provider will have available to the clients the computers needed to <br />review/research the GED program, Certificates of GED are awarded by the <br />local college upon successful completion of the program, <br /> <br />S, Vocational Rehabilitation- The provider, along with the State of North <br />Carolina Vocational Rehabilitation Services, will provide education and <br />instruction on interviewing skills, completing applications and job links and <br />services. <br /> <br />SECTION II: TERMS <br /> <br />The terms ofthis contract shall begin on July I, 2006, and, unless terminated sooner by <br />mutual consent, shall exist and continue until midnight June 30, 2007; provided that <br />either party shall have the right to terminate this contract as provided herein. <br /> <br />SECTION III: PAYMENT FOR SERVICES <br /> <br />A. In consideration for the performance of services set forth in Section I above, the <br />Provider shall receive an hourly rate of $20.00 per client treatment hour for IOPT, <br />RaPT, and Aftercare. Aftercare sessions shall be provided free of charge upon <br />request for a period up to one year after graduation. Assessments will be billed at <br />the rate of $50.00 per assessment. For billing purposes, a client treatment hour <br />must equal a minimum of forty-five minutes of service provision. <br /> <br />B. Payment for services is to be paid monthly and not to exceed $10,370 and not to <br />exceed $124,449 yearly. If in any month, the billing hours exceed $10,370 the <br />Provider agrees to continue providing services to all new and current clients, The <br />service hours not compensated for will be held until such time when money is <br />available as a result of unused funds from a lower billing month. If this does not <br />occur, the Provider agrees to donate those service hours to the program. <br /> <br />C. Provider shall submit to the NC Division of Community Corrections named <br />contact in Cabarrus County monthly invoices for actual treatment services <br />provided to said clients during the preceding thirty days.
