It is agreed that the Provider will <br />not charge for cancelled appointments or fOffaiIure-byclielltstoshow nor will the <br />Provider charge a "drop out" penalty for a client who discontinues treatment prior <br />to completing all clinical services specified in the individualized treatment plans. <br /> <br />D. Each monthly request shall include the number of hours provided according to <br />type/phase, the number of individual sessions, the number of assessments <br />conducted, the total number of clients served by phase, and the amount billed to <br />clients per month based on the breakdown of services rendered to the client. <br /> <br />SECTION IV: REPORTING AND COMPLIANCE <br /> <br />A. Provider shall provide the appropriate representative with the necessary <br />credentials to attend all client staffings, court sessions as necessary, senior <br />advisory committee meetings, conferences and training seminars. <br /> <br />B. Provider shall be required be required to provide on a monthly basis, client <br />specific information to be utilized by the court to monitor treatment compliance <br />and produce reliable outcome measures. The provider shall collect at a minimum <br />the following data: <br /> <br />Partial List of Data to be Collected: <br />I. Number of clients assessed <br />2. Number of clients entering treatment. <br />3. Number of clients obtaining each level of treatment and the number of <br />hours oftreatment per client. <br />4. Number of clients referred to alternative treatment programs_ <br />5. Number of clients completing treatment and/or each phase oftreatment. <br />6. Number of clients exiting treatment and meanslreasons for termination. <br /> <br />C. Provider shall comply with all applicable provisions of the Criminal Justice <br />Partnership Act and complete and submit all necessary forms and/or reports <br />required. <br /> <br />D. Provider shall make all treatment documentation/files available for review as <br />necessary. <br /> <br />E. The services delivered by the Provider under the terms of this contract shall meet <br />the minimum standards of providing substance abuse treatment services set forth <br />by the. North Carolina Department of Health and Human Services (DHHS). <br /> <br />F. Provider will comply with the appropriate inspection, licensing, and zoning <br />required by the State Fire Marshall's Office, as well as other applicable local <br />regulations. <br /> <br />5 <br /> <br />F-l- <br />
