<br />CONTRACT FOR SERVICES PROVIDED FOR <br />UNDER TITLE III OF THE OLDER AMERICANS ACT <br />(NC FAMILY CAREGIVER SUPPORT PROGRAM) <br /> <br />THIS AGREEMENT, entered into as of this 1st day of Julv , 2006, by and between <br />Cabarrus County Department of Aging, (hereinafter referred to as the "Agency") and the <br />Centralina Council of Governments (for services provided bv its Area Agencv on Aainal, <br />(hereinafter referred to as the "Council"); <br /> <br />WITNESSETH THAT: <br /> <br />WHEREAS, the Council wishes to have performed and the Agency wishes to provide certain <br />aging services in connection with activities financed in part by Older Americans Act grant <br />funds provided to the Council from the United States Department of Health and Human <br />Services through the North Carolina Division of Aging and Adult Services; <br /> <br />NOW THEREFORE, in consideration of the premises, and mutual covenants and agreements <br />herp.inaftF.'rcC(l!"!t21!'!ed, the parties hereto agree as follows: <br /> <br />1. Emplovment of the Agencv. The Council hereby agrees to engage the Agency and the <br />Agency hereby agrees to perform the services hereinafter set forth in accordance with <br />the terms and conditions contained herein. <br /> <br />2. Scope of Services. The Agency shall provide Family Caregiver Support services to <br />caregivers caring for individuals who are 60 years of age or older residing in <br />Cabarrus County. All services are to be performed in accordance with the Agency's <br />Caregiver Support Plan submitted to the Council, the terms of which are incorporated <br />herein by reference. In addition, the Agency agrees to perform services in full <br />compliance with NC Family Caregiver Support Program requirements and the <br />Centralina Council of Governments Area Agency on Aging Family Caregiver Support <br />Program Procedures Manual dated Julv. 2006, the terms of which are incorporated <br />herein by reference and copies of which have been furnished to the Agency. <br /> <br />3. Time of Performance. The services of the Agency are to commence no later than ~ <br />1, 2006 and shall be undertaken and pursued in such sequence as to assure their <br />expeditious completion on or before the end of the contract period, June 30. 2007. <br /> <br />4. Contract Administrator. The contract administrator for the Council shall be Gavla S. <br />Woody. Aging Program Administrator. or such other staff personnel as may be <br />designated by the Council. The contract administrator for the Agency shall be Mike <br />Murphv . Director or such other staff personnel as may be designated by the Agency. <br />It is understood and agreed that the administrator for the Agency shall represent the <br />Agency in the performance of this contract. The Agency or the Council shall notify the <br />other party if the administrator changes during the contract period. <br /> <br />5. Compensation and Method of Payment. The Agency shall be compensated for the work <br />and services actually performed under this contract by payments to be made monthly by <br />the Council. However, the total compensation and reimbursement to be paid the <br />Agency under this contract shall not exceed the sum of $ 39513 in grant funds. The <br />request for reimbursement may only include purchases in compliance with the <br />Centralina Area Agency on Aging Family Caregiver Support Program Procedures and <br />receipts for invoices must be maintained in the Agency files for review by the Area <br />Agency on Aging. The terms set forth in this agreement for payment are contingent <br />upon receipt of grant funds from the North Carolina Division of Aging and Adult <br />Services. <br /> <br />F.\~ <br />
