The Department may monitor the provider agency on-site quarterly but at least annually and more <br />frequently if the need arises to assure compliance with the terms of the contract. The Vendor Agreement <br />Provider Agencies Monitoring Report form (DSS-6849) is used in the monitoring ofthe contract. <br /> <br />8. The employee selected to provide Case Planning & Case Management services must meet the personnel <br />requirements for education and experience listed in Chapter V of the Family Services Manual. In addition, <br />this employee must be certified in a parenting curriculum conducive to existing parenting curriculums <br />utilized in Cabarrus County. <br /> <br />9. The Provider will submit weekly to the department all 'case related documentation, progress report, <br />updated assessment forms, FSCP's in accordance with state policy. <br /> <br />10. Prior to assuming responsibility of a case, the CP/CM social worker must complete state required pre- <br />service training.
