Standards for Recordkeeping. <br /> <br />Each first responder agency shall maintain the following records: <br /> <br />(I) Record of patient treatment. For each patient rendered assistance by first responder agency, <br />the following information will be recorded on a patient treatment form: <br /> <br />a. Name, age, and address Of the patient. <br /> <br />b. Date and time treatment tendered. <br /> <br />c. Location of treatment rendered. <br /> <br />d. Patient complaints and cbndition at the time of treatment rendered. <br /> <br />e. Patient vital signs and time recorded. <br /> <br />f. Treatment rendered the patient by the first responder. <br /> <br />g. Grievance pr~cedure. <br /> <br />(a) The first responder agency will be responsible for resolving grievances between its own <br />members. <br /> <br />(b) Grievances from a fitst responder agency toward the Cabarrus Ceounty Eemergency <br />Mmedical Seervices shall be submitted in writing by the first responder and co-signed by the fire <br />chief or his designee and delivered to the direeter ef eeaRl)' Eemergency Mmedical Seervices <br />Director. <br /> <br />(c) Grievances from Cabaitrus Ceounty Eemergency Mmedical Seervices personnel or Cabarrus - <br />Ceounty Board of CeoIIUI#ssioners toward a first responder shall be submitted in writing to the <br />direetElr ef eeunty Eemergency Mmedical Seervices Director who shall evaluate the grievance <br />and, if necessary relay it to'the fire chief of the appropriate agency. <br /> <br />(d) If the grievance cannot be resolved between the first responder agency and the Ceounty <br />Eemergency Mmedical Sservice a copy of the written grievance will be forwarded to Ceounty <br /> <br />r-l'5 <br />
