<br /> <br />~qlt','" COMPONENT NARRATIVE CONTINUED (attach for each component) <br />3 Describe the s cmc referral, screening, admission and tennination procedures. <br />Funding for client services will be covered by the JCPC grant; Referrals can be made throught the staff psycholgical <br />associate on site at the court house, The staff psychoiogical associate-Human Services clinician- will conduct <br />appropriate clinical evaluations for all direct referrals and coordinate additional treatment effortts at DAYMARK if the clle <br />requests such services. The staff psychological associate will assist the family in understanding how to access other <br />service providers available to them. Terminations will be based on the completion of services, compliance or <br />noncompliance with services offered, or referral to other service providers outside the DAYMARK center. This will be <br />coordinated wijh the clinician and the court counselor. <br /> <br />4) Describe im lementation to include: <br />a) The primary and supplemental services provided, if applicable: <br />Services will be available as needed. Staff psychological associate-Human Services Clinician- will provde weekly <br />appointments for assessing referrais. They will communicate with the Court Counselors regarding the most beneficial <br />contact schedule. <br /> <br />b) The means of working wijh youth and families: <br />A psycholgocial evaluation shall be conducted for each appropriate referral; this evaluation is a clinical interview in which <br />the psychological associate considers emotional, developmental, social and environmental issues that may contribute to <br />the presenting problem. Where therapeutically indicated, testing/psyc;hological assessment may be proved. Based on <br />these findings, treatment recommendations and referrals will be made. Actual delivery of services will be outside the the <br />scope of this grant; however, DAYMARK staff will be available for services including: individual, family and group therapy; <br />anger management; substance abuse treatment; Intensive Family Preservations Services; psychiatric consultations and <br />medication assessments. <br /> <br />c) The daily activities: <br />Services will be available as needed. Staff psychological associate will provide weekly appointments for assessing <br />referrals, as well as communicate wijh the Court Counselors regarding the most beneficial contact schedule. <br /> <br />F-4- <br />
