Because <br />of this, it Is important that these clients and their families receive treatment that g~s them the opportunity to succeed. <br />This entails family intervention and coordination of services. Failure of the family and the community to be Involved in <br />treatment will decrease likelihood of sucess. <br /> <br />ocumentatlon Ust the data elements or records to be maintained: A) in confidantial filas to document compliance <br />and B to document each measureble ob .ectlve. <br />All client treatment records will be maintained in compliance with NC DMH/MRISA and DJJ standards. <br /> <br />Treatment Element Describe what the component will do to redirect inappropriate youth behavior or how the <br />component will impact inappropriate behavior. <br />A psychological evaluation shall be conducted for each appropriate referral. The psychological evaluation Is a clinical <br />interview in which the staff psychological associate considers emotional, developmental, social and environmental Issues <br />that may contribute to the presenting problem. Where therapeutically indicated psycholgicai assessmentltesting may be <br />provided. BAsed on these findings, treatment recommendations and referrals will be made. Actual delivery of services <br />will be outside the scope of this grant; however, DAYMARK staff will be avaiaibe for sercias including: individual, family <br />and group therapy; anger management; substance abuse treatment; Intensive Family Preservation Services; pSYChiatric <br />consultations and medication assessment. <br /> <br /> <br />COMPONENT NARRATIVE (attach fat each component) <br />Component Narrative (attach for each component) <br />(1) Describe the physical facilities, inclUding the address, in which the sarvices will be delivered. If seIVices are <br />delivered at multiple locations, please include information for all locations. <br />Cabarrus County Court House, Union Street, Concord, NC is the site for these services for the primery grant position <br />held by the staff pSYChological Associate- Human Services Clinician. DAYMARK is the site of all other service delievery if <br />the client and family is referredfot and chooses additional services not covered by the grant. <br /> <br />(2) Identify paid or volunteer staff qualifications, (certi cations, degrees, work experience) and their responsibilities <br />relative to this com anent. <br />Clinical Site Director: supervises all aspects of the clinical services. Team Leader provides day to tday supervision to all <br />stsff associated with the DJJDP Program. JCPC Psychoiogical Associate-Human Services Clinician- Maste(s Ievei <br />clinician, provides on site psychoigical services delievery to DJJDP clients and their families, and coordinates referrals <br />and consuitstions to the court. DAYMARK staff providing services to referred clients hold a minimum of a Master's <br />Degtee in related mental heaith field and hold a Certified Clinical Addictions Specialist certification or are in the process 0 <br />receiving supervision for this certification.
