<br />.1'~E!:I'llPI\nIU ~l!tJ!@fm COMPONENT SUMMARY CONTINUED (attach for each component) <br />Areas of Concern Discuss any arees of concern that could affect success. <br />Youth referred to service generally have a long history of acting out, family dysfunction and substance abuse. Because <br />of this, it is important that these clients and their families receive treatment that gives them the opportunity to succeed. <br />This entails family intervention and coordination of services. Failure of the family and the community to be involved in <br />treatment will decrease likelihood of sucess. <br /> <br />Documentation Ust the data elements or records to be maintained: A) in confidential files to document compliance <br />and B to document each measurable ob 'ective. <br />All client treatment records will be maintained in compliance with NC DMHIMRtSA and DJJ standards. <br /> <br />Treatment Element Describe what the component will do to redirect inappropriate youth behavior or how the <br />component will impact inappropriate behavior. <br />A Substance Abuse evaluation shall be conducted for each appropriate referral. This evaluation is a clinical interview in <br />which the Substance Abuse clinician considers emotional, developmental, social and environmental issues that may <br />contribute to the presenting problem. Based on these findings, treatment recommendations and referrals will be made. <br />Actual delivery of services will be outside the scope of this grant, however, DA YMARK staff will be avaialbe for sereies <br />including: individual, family and group therapy; anger management, substance abuse treatment; Intensive Family <br />Preservation Services; psychiatric consultations and medication assessment. <br /> <br />'.. . COMPONENT NARRATIVE (attach for each component) <br />Component Narrative (attach for each component) <br />(1) Describe the physical facilities, including the address, in which the services will be delivered. If services are <br />delivered at multiple locations, please include information for aI/locations. <br />Cabarrus County Court House, Union Street, Concord, NC is the site for these services for the primary grant position <br />held by the staff psychological Associate- Human Services Clinician. DAYMARK is the site of all other service delievery <br />if the client and family is referred for and chooses additional services not covered by the grant <br /> <br />(2) Idenlify paid or volunteer staff qualificalions, (cartificalions, degrees, work experience) and their responsibiliUes <br />relative to this com nent. <br />Clinical Site Director: supervises all aspects of the clinica! services. Team Leader provides day to tday supervision to all <br />staff associated with the DJJDP Program. JCPC Psychological Associate-Human Services Clinician- Maste(s level <br />clinician, provides on site psycholgical services delievery to DJJDP clients and their families, and coordinates referrals <br />and consultations to the court OA YMARK staff providing services to referred clients hold a minimum of a Maste(s <br />Degree in related mental health fiBld and hold a Certified Clinical Addictions Specialist certification or are in the process of <br />receiving supelVision for this certification. In addition, clients of DA YMARK have access to psychiatric selVices. <br /> <br />F.4- <br />
