<br />I IV-A (contl I COMPONENT NARRATIVE CONTINUED (attach for each component) <br />3) Describe the specific referral, screenina, admission and tennination procedures. <br />100% of the participants are Juvenile Court and Juvenile Court Counselors referred. All referrals are screened by <br />the Juvenile Court Counselors for the appropriateness of our program. Project Challenge provides diversion, Levell, <br />and Level II services to the Juvenile Court. Once the referral has been made, an intake process is performed. This <br />process includes completing appropriate applications, medical and emergency contact information and all pertinent <br />program related information. At this time both the participant and the parent/guardian is informed of the roll Project <br />Challenge plays and what is expected from both the child and guardian. <br /> <br />(4) Describe imo/ementation to include: <br />a) The primary and supplemental services provided, if applicable: <br />Project Challenge's ptimary service is Restitution. <br /> <br />b) The means of working with youth and families: <br />Staff/participant interactions occur during community service and wildemess--trip activities. Ptogram Coordinators <br />inform families of participant ptogress and rely on familial input to evaluate participant's success at home. Each youth <br />has an individual program plan that describes the goals and time line with which the youth must comply. <br /> <br />c) The daily activities: <br />Daily program activities include performing community service at a variety of sites. These include nursing homes, <br />food pantries, animal shelters, parks and recreation departments, literacy councils, civic agencies and individuals in <br />need within a community. During the school year, these activities take place after school hours, and on Saturdays. <br />Daily program activities may also involve participation the wilderness adventure component of Ptoject Challenge.' <br /> <br />Page 5 of 10 <br />t: -4- <br />
