<br />Individuals who need time-limited support in making the transition from <br />independent living to group care, or -individuals who need time-limited support <br />in making the transition from group care to independent living. <br /> <br />The Adult Day Health Specialist through the Health Department will monitor the <br />facility on a quarterly basis and notify the Cabarrus County Department of <br />Social Services when problems of non-compliance arise. <br /> <br />The Adult Day Health Specialist and the County Adult Day Care Coordinator will <br />have two joint monitoring visits per year. <br /> <br />The Adult Day Health Specialist will submit a DSS-6205-B (Adult Day Care/Day <br />Health Certification Report) to the Cabarrus County Department of Social <br />Services for the initial certification of the facility and annually thereafter. <br /> <br />Services to he ~rovided: <br /> <br />(1) <br /> <br />(2) <br /> <br />(3) <br /> <br />(4) <br /> <br />(5) <br /> <br /> Number of Number of Definit.ion of <br />Services Service Code Persons Served Units of Unit of <br /> Service Service <br />Adult Day Health 155 6.2 1481 1 unit -1 day <br /> . <br /> <br />(X) a. a standard fixed rate of $750.00 per month for care or 38.00 per day. <br /> <br />(I-I) b. an individual rate. <br /> <br />Reimbursement will be made at a fixed rate of $38.00 per unit of Day Health Care <br />(define) for an estimated number of 1481 units. Reimbursement will be based on <br />the actual number of units delivered whether over or under the estimated number. <br />If multiple components are being purchased, additional rates and units can be <br />identified as follows: $ 3.00 per unit of day care transportation for an <br />estimated number of 2962 units. <br /> <br />*Any additional service should be listed in the Service Program Description. <br /> <br />ADULT DAY HEALTH PROGRAM <br /> <br />The Adult Day Health program is to provide a protective setting, supportive and <br />health related services to help meet the need of the adult participant and thus <br />reducing the need for further intensive supervision, health care or placement <br />services. The Adult Day Health environment would also prOVide socialization, <br />stimulation and enrichment services which would enhance the participants stay in <br />their home and community. <br /> <br />The maximum allowable capacity for participants in a combination program (Day <br />Care/Day Health) is 50 square feet. The maximum is 60 square feet per <br />participant in an Adult Day Health program. <br /> <br />A minimum of 25% of the participants in daily attendance in a combination <br />program must be enrolled for Adult Day Care services. <br /> <br />1=-<'.:, <br />
