<br />If it is determined that normal or modified participation will resume, the <br />enrollment will be kept active. If participation by the enrollee has not resumed <br />within a maximum of thirty (30) calendar days, enrollment will be terminated. <br /> <br />B. If it is determined that normal or modified participation will not resume, <br />enrollment will be terminated immediately and payment will cease. <br /> <br />The Cost-Share amount has been determined based upon the client's ability to <br />pay. The Cost-Share amount should be made on a timely basis (within 10 calendar <br />days of the due date) or reasonable efforts should be made to collect Cost-Share <br />revenue at least quarterly. When payment is not received, a written notice for <br />the agreed amount must be sent to the client with a copy to the social worker <br />with the county Department of Social Services who is responsible for the <br />management of the case. <br /> <br />The responsibility for collecting and accounting for Cost-Sharing revenue is the <br />responsibility of the Coltrane Life Center. <br /> <br />Services to the client will not be terminated for failure to share in the Cost <br />of SerVices rendered. <br /> <br />Effective 7/1/89, eligibility for adult day care will be determined on the basis <br />of need. Only those individuals who need the service for a minimum of six (6) <br />hours a day should be considered for adult day care services. Once and <br />individual is determined to be eligible for the service, it shall be provided <br />according to the following order of priority: <br /> <br />Adults (preference given to .59 years of age and under) who require complete, <br />full-time Daytime supervision in order to live in their own home or prevent <br />impending placement in substitute. care, and adults who need the service as part <br />of a protective services plan. <br /> <br />Impending placement in substitute care means that the client's physician has <br />recommended skilled nursing, intermediate, or domiciliary home care; or the <br />client has applied for admission to such a facility; or the client's family <br />states that placement in such a facility is about to occur. <br /> <br />Adults (preference given to 59 years of age and under)who need help for <br />themselves with activities of daily living and/or support for their caregivers <br />in order to maintain themselves in their own homes. <br /> <br />Adults (preference given to 59 years of age and under) who need intervention in <br />the form of enrichment and opportunities for social activities in order to <br />prevent deterioration that would lead to placement in group care. <br /> <br />Intervention in the form of enrichment and opportunities for social activities <br />is directed toward individuals who are isolated and withdrawn and who, because <br />of their withdrawal, are in danger of deteriorating mental and physical health. <br />Individuals who are .lonely and who could benefit from planned social and <br />recreational activities, but who are able to function independently, should. be <br />referred to an organization designed to meet such needs, not to adult day care. <br /> <br />&b <br />
