<br />Family Services <br />State of North Carolina <br />Division of Social Services <br />Contract Application <br /> <br />Page 9 of 10 <br /> <br />III Service Program Description <br />Specified below is the service to be provided by the Vendor under this agreement. <br /> <br />1. The provider agrees to provide Case Planning & Case Management Services to families who have been <br />certified as eligible by the Department for Child Welfare Block Grant Services. The provider agrees to <br />maintain records to document the provision of services for three years, or until audits are completed, or <br />until the youngest child in the family reaches age 18, whichever occurs later. The Provider will furnish a <br />monthly report (DSS-1571, Part IV) and the DSS Daysheet, DSS 4263 to the Department in order that <br />the request for reimbursement may be submitted no later than the 10th working day of the month following <br />the provision of service. Both the DSS-1571, Part N and the DSS Daysheet, DSS 4263 are to be <br />submitted the first working day of each month. <br /> <br />2. The Department agrees to determine the clients' eligibility for Case Planning & Case Management Services <br />in conjunction with the Child Welfare Block Grant Services. All parenting instruction will occur in the <br />family's home. Referrals will be received from the Department of Social Services Investigation unit or as a <br />result of a CP/CM case transfer for another county within North Carolina. The provider will collaborate <br />with the department by offering CP/CM services (when possible) to families with mental health issues. The <br />provider is equipped with resources, knowledge, and expertise necessary to address CP/CM cases that <br />involves complex mental health problems. These cases/families would benefit from having a trained <br />professional that is intensively involved with both systems (Child Protection & Mental Health). <br /> <br />3. Case Planning & Case Management Services - The purpose is to maintain the safety of the child while <br />helping the parent/caretaker to learn more effective parenting practices. CP/CM services are offered after <br />a case substantiation of abuse, neglect, or dependency. The primary concern continues to be the child's <br />safety and protection. The department is responsible for making reasonable efforts to maintain the child <br />safely in the home, but when safety in the child's home cannot be assured, the department is mandated to <br />take inunediate action to protect to the child by providing safe, nurturing substitute care. CP/CM social <br />workers thus maintain a single focus while preparing for two different outcomes. <br /> <br />4. Activities include counseling to enable the individual to recognize, understand, and cope with problems <br />and conflicts in regard specifically to learned parenting styles and parent/child interactions in the following <br />areas: effective discipline strategies; age appropriate expectations and child development; consistent <br />expectations and consequences; supervision issues; effective connnunication; preventive health and dental <br />care; safety precautions in the home; empathetic awareness of children's needs; stress/anger management <br />techniques. The CP/CM social worker often makes referrals to external resources and a significant role of <br />the program is to case manage services to the family. This involves connecting the family with needed <br />resources. <br /> <br />5. Counseling is aimed at enabling the client to recognize, understand and cope with his particular problems <br />and conflicts. In the context of this family-centered approach, counseling is a process of establishing a <br />relationship in which information, ideas, and opinions can be mutually exchanged; decision-making options <br />explored; and positive action toward problem resolution initiated. Because. of the case management <br />component of this program, the CP/CM social worker must at times rely on other resources to assist with <br />the provision of services. <br /> <br />r - ':)J <br />
