Oral presentations are an option of Cabarrus County and mayor may not <br />be conducted. <br /> <br />SPECIFIC REQUIREMENTS: <br /> <br />Proposals should be as thorough and as detailed as possible so that Cabarrus County may properly <br />evaluate your capabilities to provide the required services. <br /> <br />1. Offerors are required to submit information as to authority to conduct business in the State of North <br />Carolina, banking information and contact person. <br /> <br />2. A description of Offeror's history in providing required services. <br /> <br />3. Listing of Offeror's management and staff personnel to be used for this project detailing <br />qualifications and experience relative to the services described herein. <br /> <br />4. Submit any other information that you believe is relevant in evaluating your proposal. <br /> <br />By submitting their proposals, all Offerors certify that their proposals. are made without collusion or fraud <br />and that they have not offered or received any kickbacks or inducements from other Offeror in connection <br />with their proposal. <br /> <br />. All state/county licenses and certifications shall be kept in accordance with the iaw or with the show <br />office. <br /> <br />. All rides shall be inspected in accordance with the requirements of NCDOL (North Carolina <br />Department of Labor). <br /> <br />. All subcontracted rides shall follow the same rules and regulations as the contracted show. <br /> <br />PERSONNEL: <br /> <br />. Training, communications, the work environment, housing, plus fair and equitable treatment of <br />employees are critical for a carnival. <br /> <br />Empioyees shouid know regulatory requirements of OSHA (Occupational Safety and Health <br />Administration) pertaining to their job skills. <br /> <br />. Salary, hourly wages, and benefits should be commensurate with skill level attained and performance <br />guidelines of the show owner. <br /> <br />. Hiring and employment of personnel must adhere to state and federal regulations. Provide <br />procedures for performing background checks on both permanent and temporarY employees. <br /> <br />Employees should be educated in OSHA regulations and shall be prOVided with appropriate personal <br />protective equipment to do their jobs safely. <br /> <br />. Establish "Code of Conduct" for proper employee behavior and/or consequences for bad behavior, up <br />to arid. including termination of employment - attach a copy of policy to proposal. Include training <br />procedures that emphasize youth protection. <br /> <br />. All employees must be hired with proper documentation for federal and state tax purposes (i.e., <br />Social Security number, W-4 form, INS form 1-9, etc.) <br /> <br />An individual personnel file should be established for each full-time empioyee with pertinent individual <br />information, such as employment application, payroll information, emergency notification, baCkground <br />investigation or references, disciplinary actions, job history, performance reviews, training programs <br />completed, salary/wage history, etc. <br /> <br />r;~/O <br /> <br />3 <br />
