COMPONENT SUMMARY (Attach for each component) <br />Statement of the Problem(s): In concise terminology, describe the juvenile just/ce problem(s) the program <br /> <br />intends to address and how these services meet the needs identified in the JCPC Annual Plan. <br />Cabarrus County Department of Social Services, hereinafter referred to as CCDSS, has experienced an <br />increased level of intensity in the behaviors and needs of juveniles referred for out of home placement from the <br />Department of Juvenile Justice, hereinafter referred to as DJJ. Many of these juveniles lack family <br />involvement/support, lack proper parental supervision, have increased drug/alcohol usage, have increased <br />school difficulties, and many are becoming increasingly involved in criminal activities resulting in felony <br />charges. The level of care needed to deal with these behaviors and needs can only be found in specialized <br />residential treatment facilities geared toward providing long term treatment in an effort to reduce further <br />involvement of these juveniles within the juvenile justice system. These treatment facilities are typically <br />referred to as Level III or Level IV placements depending on the level of care provided. <br /> <br /> Priority Risk Factor(s) Addressed: State how the program will address the priority risk factor(s) identified by <br /> the JCPC. <br />Placement in sp.ecialized residential treatment facilities provides a structured out of home placement with <br />behavioral modification and counseling for each juvenile. Each treatment facility focuses on establishing a <br />structured daily routine which includes school attendance in the public school setting or on-campbs setting, <br />daily living responsibilities, group sessions to deal with personal relationships, and access to professional <br />counseling sessions to assist juveniles in dealing with family relationships which may have contributed to the <br />out of home placement. Each juvenile and their family (if available) will work with a CCDSS social worker to <br />develop a Family Services Case Plan addressing the factors that resulted in the out of home placement. <br />Areas addressed in the plan may include, but are not limited to, parenting classes, individual and/or family <br />therapy, visitation between the juvenile and their family, independent living skills, employment, education, etc. <br />This plan is forwarded to the residential treatment facility in an effort to have a coordinated approach toward <br />eliminating the juvenile's further involvement in the juvenile justice system as well as providing the best <br />opportunity toward reunification of the juvenile back into the home. Each residential treatment facility provides <br />close supervision of the juvenile's activities and interactions with other juveniles. Anytime additional needs <br />arise, the facility and/or CCDSS will make prompt referrals to the necessary resources. <br /> <br />Program Goals: State the effect that the program is designed to have on solving tJ~e problem(s) described <br />above. <br />Placement in specialized treatment facilities will provide a non-secure and/or secure (depending on the level of <br />care needed) out of home placement for the juveniles who are in court for undisciplined and/or delinquent <br />behaviors or those juveniles at risk of entering the juvenile justice system. Each treatment facility will provide a <br />structured placement with behavior modification, counseling, coordination of services and case planning in an <br />effort to stabilize negative behaviors, decrease drug/alcohol usage and/or abuse, improve self-esteem, <br />improve school performance, and stabilize family dynamics. Intervention services provided by these <br />specialized treatment facilities will result in a decrease in the undisciplined/delinquency rate of juveniles in <br />Cabarrus County. <br /> <br />Target Population: Describe the target population and the steps that the program has taken to insure that the <br />target population is served. <br /> <br />The target population will be juveniles residing in Cabarrus County between the ages of 10 to 17 who are <br />involved in the juvenile justice system, or who are at risk of becoming involved, to the degree that out of home <br />alacement is imminent. Early coordination between CCDSS and DJJ will occur in an effort to secure the most <br />appropriate placement of each juvenile. Once the juvenile is placed, there will be a continual flow of <br />information between CCDSS, the juvenile court counselor, and the treatment facility to ensure that each <br />juvenile is receiving adequate and effective treatment. <br /> <br />JCPC PROGRAM AGREEMENT, 2004-2005 <br /> <br /> <br />
