Measurable Objective(s): State in measurable terms the intended effect of the program on specific <br />undisciplined and/or delinquent behaviors. At a minimum, state anticipated reductions in court referrals, <br />runaway behavior, disruptive behavior at school, anticipated improved school attendance and academic <br />achievement. These objectives must include participant outcomes and may include program outcomes. <br />1. Program will serve at least 8 youth referred by the Department of Juvenile Justice during the fiscal year <br />2004-2005. <br /> <br />2. 75% of the youth served will successfully complete the areas addressed on the Family Services Case Plan <br />prior to termination from the program. <br /> <br />3. 75% of the youth served will decrease involvement in the juvenile justice system as measured by recidivism <br />in technical violations and reduction in new crimes. <br /> <br />Information Maintained for Effectiveness Measurement: List the data elements and records the program <br />will maintain to document its effectiveness. <br />A record will be kept on each juvenile by CCDSS as well as by the treatment facility which will contain, but not <br />limited to, an admission application, the juvenile's birth certificate, social security card, and immunization <br />record, any medication orders signed by the prescribing physician, a social history, visitation <br />restrictions/guidelines, school information, medical/psychological information, emergency contact information, <br />court orders resulting in the out of home placement and subsequent court information, plan of care staffing <br />information, case plans, etc. A detailed log will be maintained by CCDSS tracking each juvenile's placement, <br />cost of care, and outcome of the treatment received. <br /> <br />JCPC PROGRAM AGREEMENT, 2004-2005 <br /> <br /> <br />
