COMPONENT NARRATIVE (Attach for ea(3h component) <br />Bdefly describe what the program expects to achieve and why. The narrative should address each of the <br />following items (1) Identify the staff and their responsiblTities to the program, (2) describe the physical facilities <br />in which the services will be delivered, (3) discuss any areas of concern that could affect the success of the <br />program, (4) discuss why these resources used in the manner described should be expected to alter <br />inappropriate behavior, (5) describe the referral, screening and admissions procedures, (6) describe the <br />means of working with youth and families, (7) describe the protective factors the program will use to address <br />the identified risk factors, (8) describe the daily program activities, (9) describe the termination procedures, <br />and (10) describe the interaction with juvenile court. <br />1. Clinical Site Director: supervises all aspect of clinical services; Team leader provides day-to-day <br />supervision to all staff associated with the DJJDP program. JCPC psychologist provides on-site psychological <br />services delivery to DJJDPclients and their families, and coordinates referrals and consultatins to the court. <br />DAYMARK staff providing services to referred clients are both mental health and substance abuse <br />professionals. In addition, clients have access to psychiatric services. <br /> <br />2. Physical Facilities: Cabarrus County Court House is the site of direct service provision for the primary grant <br />position held by the psychologist. DAYMARK is the site of all other service delivery if the client and family is <br />referred for additional services not covered by the grant. <br /> <br />3. Areas of Concern: Youth referred to service generally have a long history of acting out, family dysfunction, <br />and substance abuse. Because of this, it is important that these clients and their families receive treatment <br />that gives them the opportunity to succeed. This entails family intervention and coordination of services. <br />Failure of the family and the community to be involved in treatment will decrease likelihood of success. <br /> <br />4. Funding for half-time psychologist position at court house insures timely access to the clinician for <br />screenings, assessments, testing, and consultation. This facilitates good communication, timely decision <br />making and increased therapeutic efficacy. Referrals for additional services can be made for needed services <br />such as parenting, family therapy, individual therapy group therapy, and psychiatric services. This continuim of <br />SA/MH services increases the likelihood of treating the child and family effectively. <br /> <br />5. A. Funding for client services will be covered by the JCPC grant. <br /> B. Referrals can be made through the psychologist on site at the court house. The psychologist will <br />conduct clinical evaluations for all direct referrals and coordinate additonal treatment efforts at DAYMARK for <br />the client. The psychologist will assist the family in understanding how to access other service providers <br />available to them. <br /> C. Terminations will be based on the completion of service, noncompliance with services offered, or <br />referral to other services outside the DAYMARK Center. This will be coordinated with the clinician and the <br />court counselor <br /> <br />6. Services will include clinical assessments as well as psychological evaluations where clinically indicated. <br />Based on these findings, further treatment recommendations will be made, and the client will be referred to <br />appropriate therapeutic services. Actual delivery of services will be outside the scope of this grant; however, <br />DAYMARK staff will be available for services including: individual and family therapy; group therapy; anger <br />management; substance abuse treatment; Intensive Family Preservation Services; psychiatric consultations <br />and medication assessments. <br /> <br />7. Protective factors to address "risk factors" will include a comprehensive assessment with specific treatment <br />recommendations focusing on increasing the child and the family's protective factors while decreasing the <br />impact of the risk factors the family is facing. <br /> <br />8. Services will be available as needed. Psychologist will provide weekly appointments for assessing referral. <br />The psychologist will communicate with the court professionals regarding most benefical contact schedule. <br /> <br /> <br />
