· I "SECTION IV t <br /> <br /> COMPONENT NARRATIVE (Attach for each component) <br />Briefly describe what the program expects to achieve and why. The narrative should address each of the <br />following items (1) Identify the staff and their responsibilities to the program, (2) desc#be the physical facilities <br />in which the services will be delivered, (3) discuss any areas of concem that could affect the success of the <br />program, (4) discuss why these resources used in the manner described should be expected to alter <br />inappropriate behavior, (5) describe the referral, screening and admissions procedures, (6) describe the <br />means of working with youth and families, (7) describe the protective factors the program will use to address <br />the identified #sk factors, (8) descdbe the daily program activities, (9) describe the termination procedures, <br />and (10) describe the interaction with juvenile court. <br />1. Program staff includes Clinical site director, who supervises all aspects of center services; C&Y Team <br />leader, who provides supervision to all staff assoicated with the DJJDP program. DAYMARK SA professional <br />provides on-site SA service delivery to DJJDP clients and their families, and coordinates referrals and <br />consultations to the court. Staff providing services to referred clients at DAYMARK Center include both mental <br />health and substance abuse professionals. This staff delivers a variety of services including SA and MH <br />treatment. Psychiatric services are available. <br /> <br />2. Physical Facilities: Cabarrus County Court House is the site of direct service provision for the primary grant <br />position held by ihe SA therapist. DAYMARK is the site fo all other service delivery if the client and family is <br />referred for additional services not covered by the grant. <br /> <br />3. Areas of concern: Youth referred to services generally have a long history of acting out, family dysfunction, <br />and substance abuse. Because of this, it is improtant that these clients and their families receive treatment <br />that gives them the opportunity to succeed. This entails family intervention and coordination of services. <br />Failure of the family and the community to be involved intreatment will decrease likelihood of success. <br /> <br />4. Funding for the half-time SA position at the court house insures timely access to the clinician for <br />assessments, testing, and consultation. This facilitates good communication, timely decision making and <br />increased therapeutic efficacy. Referrals for additional services can be made as needed such as parenting, <br />family therapy, individual therapy, group therapy, and psychiatric services. This continuim of SA/MH services <br />increases the likelihood of treating the child and family effectively. <br /> <br />5. A. Funding for client services will be covered through the JCPC grant. <br /> B. Referrals can be made through the SA therapist on site at the court house. SA therapist will conduct <br />screeings for all direct referrals and coordinate additional treatment efforts at DAYMARK if appropriate. <br />C. Terminations will be based on the completion of service, noncompliance with services offered, or <br />referral to other services outside DAYMARK. This will be coordinated with the clinician and court counselor. <br /> <br />6. Services will include screenings and SA assessments. Based on these findings, further treatment <br />recommendations will be made, the client will be refered to appropriate therapeutic services. Actual delivery <br />of services will be outside the scope of this grant; however, DAYMARK staff will be available for services <br />including: SA Outpatient services, individual and family therapy; group therapy; anger management; <br />substance abuse treatment; Intensive Family Preservation Services; psychiatric consultations and medication <br />assessments. In addition, MAJORS and PREVENTION programs, which are separate funds, may be <br />recommended. <br /> <br />7. Protective factors to address "risk factors" will include a comprehensive asessment with specific treatmnt <br />recommendations focusing on increasing chid and the family's protective factors while decreasing the impact <br />of the risk factors the family is facing. <br /> <br />8. Services will be available as needed. SA therapist will provide weekly appointments for testing and <br />screening/assessment. Additionally, SA therapist will communicate with court personnnel regarding most <br />effective scheduling. <br /> <br /> <br />
