<br />. <br /> <br />Checking with references; <br /> <br />. <br /> <br />Completing documentation; <br /> <br />. <br /> <br />Staffing finding of Home Study with Supervisor with the <br />Department and Provider for decision on the appropriateness of <br />placement (approval or not); <br /> <br />. <br /> <br />Writing formal report to court; and <br /> <br />. <br /> <br />Attendance and participation in juvenile court as needed. <br /> <br />1. <br /> <br />PLEASE NOTE: <br />Time spent in preparing documentation and reasonable documented travel <br />time are billable. <br /> <br />2. <br /> <br />.1 <br />Reimbursement will not be pro-rated. Reimbursement will only include <br /> <br />actual time billed for 40 hour work weeks. <br /> <br />3. Time spent in staff development and training are not billable. <br /> <br />D. <br /> <br />Audit Requirements: <br /> <br />1. <br /> <br />An annual audit is to be performed in accordance with OMB <br />Circular ]>,_-110 by an "independent auditor". "Independent <br />Auditor" means either: (a) a state government auditor <br />from the Depart~ent of Human Resources or the Department of <br />Administration, Office of the State Auditor; or (b) a <br />certified public accountant. Upon completion of the audit, a <br />copy of the audit report must be forwarded to the Department. <br /> <br />E. <br /> <br />Client Fees for Service: <br /> <br />1. <br /> <br />No fees will be charged to individuals determined to be eligible for <br />Services by the Department. <br /> <br />F. <br /> <br />Management of the Funding/Matching Share Requirement: <br /> <br />1. <br /> <br />The Department is providing the match. <br /> <br />III. Service Program Description <br />Specified below are the Services to be provided by the Provider under this <br />agreement. <br /> <br />1. The Provider agrees to provide Home Studies/Assessments to all County <br />families who have been certified as eligible by the Department for Child <br />Welfare Block Grant Services. The Provider agrees to maintain records to <br />document the provision of Services for three years or until audits are <br />completed, whichever occurs later. <br /> <br />2. Home Studies/Assessment will be completed on different types of <br />requests, to include referrals from Child Protective Services and Foster <br />Care Social Workers, and referrals from other counties and states. <br /> <br />r-4 <br />
