The position will be housed within the walls of the <br />Departmen t . <br /> <br />B. <br /> <br />Method of Reimbursement: <br />a. The Department will pay the Provider monthly for twelve months for a <br />total of $55,000.00. Reimbursement, cannot be claimed for Services <br />in absence of authorization by the Department (DSS-5027 form). <br /> <br />The Provider must allow for annual and sick leave, staff development <br />time, etc. <br /> <br />The Provider will conduct an internal audit of this contract <br />quarterly. <br /> <br />The Provider will complete monthly daysheets that reflect the amount <br />of social worker time spent on each family contact. <br /> <br />b. <br /> <br />Billable time for Home Studies. When billing for payment, Provider <br />may include the time spent by the Provider employee in performing <br />the following activities: <br /> <br />. <br /> <br />Reviewing the Initial Home Study referral form; <br /> <br />. <br /> <br />Staffing with the referring Social Worker and/or Supervisor; <br /> <br />. <br /> <br />Setting up Home Study Record when needed; <br /> <br />. <br /> <br />Completing criminal record and central record checks on all <br />household members; <br /> <br />. <br /> <br />Scheduling home visit with the family, to include a minimum of <br />two visits to the home; <br /> <br />. <br /> <br />Completing Economic Assessment sheet on the family; <br /> <br />F-t <br />
