<br />The Day Health program must be responsible for providing therapeutic diets as <br />needed for the participants. Medical information from the participants' medical <br />provider will specify the type of diet the participant should have. <br /> <br />The Program Director and Health Care Coordinator can be the same person only in <br />programs with 10 or fewer participants. <br /> <br />In facilities where the Realth Care Coordinator is a Licensed Practical nurse, <br />supervision must be provided by a Registered Nurse on-site no less than every <br />two weeks. <br /> <br />Medical information should be kept on each participant in the Day Health program <br />and the participants should be monitored for any unusual behavior or changes in <br />their health that may suggest a need for help or further medical attention. <br /> <br />The Adult Day Health facility must have a treatment room that provides privacy <br />during health care serviGes. Ail required medical equipment and supplies must be <br />maintained. The quiet space for the participants to lie down as needed must be <br />separate from the treatmènt room. <br /> <br />Assist the participants with activities of daily living including, feehing, <br />ambulating and toileting. Health care monitoring should be assessment of the <br />vital signs, weight, dental health, general nutrition and hygiene along with <br />health educational programs for the participants on a regular basis. <br /> <br />Specialized services such as speech therapy, physical therapy and counseling <br />service as required by a physician and available through community resources. <br /> <br />1. Individuals enrolled in an adult day care/day health program who no longer <br />are able to maintain themselves in an independent living situation and for whom <br />placement in group care is necessary may continue attending the adult day <br />care/day health program for a maximum of ninety days after entering the group <br />care facility. This time-limited participation in adult day care is for the <br />purpose of providing support during the period of adjustment of the new living <br />situation. State and federal funds may not be used to support the provision of <br />adult day care/day health for individuals in group care beyond this ninety-day <br />,period. , <br /> <br />2. Adult day care/day health services may be provided for an individual <br />preparing to leave a group care facility for an independent living arrangement <br />.for a.period..of up to.ninety.days-prior to.the indivi.dual's.discharg.e-from.the <br />group care facility. Adult day care/day health in this instance is intended to <br />assist the individual in preparing for independent living and for the move from <br />the group care facility. The individual must be eligible after discharge from <br />the group care facility in order to continue to receive the service. <br /> <br />F-~ <br />
