Steps to Utilize Additional Slots: <br /> <br />. <br /> <br />We will process referrals as they are received when the FL-2 arrives. When referrals <br />slow down, we will remind our community partners (hospitals, home health agencies, <br />home care agencies, and physicians' offices) that we have slots available to assist their <br />patients. <br />One new position has been requested. The agency director has been receptive to adding <br />staff. A budget amendment will be taken to our County Commissioners in October 2004. <br />With the new case manager, the two case managers processing referrals will have other <br />duties. One will continue to process cases full time with a small caseload and the other <br />will carry a small caseload and assist in local approval. The latter will be a floating case <br />manager. Caseload growth and number of referrals received will be monitored to <br />determine when the second case manager is needed. <br />We will continue to process referrals as soon as the FL-2 is received. Currently, a list is <br />maintained of all persons who apply. A clerk mails letters to each applicant every <br />Monday to request an FL-2 if it did not accompany the referral. The referral is requested <br />within a two week period. The case managers processing referrals always has a <br />telephone contact with all persons who apply to assure that the process is understood and <br />to offer assistance if necessary. <br /> <br />. <br /> <br />. <br /> <br />. <br /> <br />B. Timeline for Slot Utilization <br /> <br />It is anticipated that three new cases can be added each month. If referrals slow down, the <br />two case manager processors will make presentations to local agencies to assure they are <br />aware that we are able to take on new cases. We will also do news releases through our <br />County Information Officer to assure the public is aware with have CAP slots available with <br />no waiting list. , <br />We hope to have added 30 cases by June 2005.
