Ifwe are able to add new positions, <br />we will continue to add persons to the program. On occasion, referrals slow down. <br />When this occurs we will contact home health agencies, home care agencies, <br />physicians, and the hospital to assure they are aware ofthe valuable services CAP/DA <br />can provide. <br /> <br />D. Future Plans <br /> <br />Ifwe are able to continue to have a net gain of cases, we anticipate having an active CAP/DA <br />caseload of269 by June 2005.
