LOCATIONS <br /> <br />The physical address of the location(s) where service(s) is/ are to be terminated is/are <br />specified in the Attachment(s) to follow. <br /> <br />3. LENGTH OF AGREEMENT <br />The Customer agrees to pay the Company for the service(s) covered under this <br />Agreement at rates in effect for the minimum commitment period noted on the <br />Attachment(s) to follow. The Company guarantees that there will be no Company <br />initiated rate change for the requested service( s) during the minimum commitment <br />period. Rates may change, with regulatory approval, on such additional charges as 911, <br />Subscriber Line Charge, Dual Party Relay and Federal Universal Service Charge. Billing. <br />will commence the day the service(s) requested is/are installed. However, other rates <br />applicable to additional services provided by the Company which may be\,1sed in <br />connection with the requested service( s) may be changed during the minimum <br />commitment period as governed by the respective tariff. <br /> <br /> <br />4. TERMINATION <br />In the event that any item covered in this Agreement is terminated prior to the end of the <br />minimum commitment period, the Customer shall pay a Termination Liability charge as <br />specified in the Company's GENERAL EXCHANGE TARIFF. The following <br />conditions will be treated as a termination of service: <br /> <br />A Removal of the service(s) when requested by the Customer. <br /> <br />B. Moving to a new location if the critería for "move of service" in the General <br />Exchange Tariff's Termination Liability are not met. <br /> <br />1 <br />F.17 <br />
